The Primary Care Nurse (PCN) is part of an interprofessional team, providing safe, ethical care in various settings like physician practices, homes, schools, and communities. They follow up on clinical care based on the person's Care Plan and personal goals, collaborating with the team to develop care plans. The PCN offers direct clinical care to individuals, groups, and communities.
Primary Care Networks in BC are collaborations between family physicians, regional health authorities, the First Nations Health Authority, and local community partners, including nurse practitioners. They aim to improve the primary and community care system for patients and health-care providers.
