Provides liaison support for patient care and patient flow between the PEAT program, Emergency Department, Mental Health Inpatient Unit, first responders, community health care providers and community partners/agencies including school counselors. Provides direct care to mental health and/or substance use patients awaiting transfer for further assessment or inpatient admission. Initiates and participates in case conferences and case management meetings, prepares and maintains patient records and provides consultative and direct services/care to patients, families, community health professionals/agencies, law enforcement, physicians and emergency room staff. The Psychiatric Emergency and Assessment Team (PEAT) provides the residents of the North Shore and surrounding areas with access to prompt psychiatric assessments through the Emergency Department at Lions Gate Hospital.
The program also expedites the care and appropriate triaging of psychiatric patients in the Emergency Department. Additionally, it provides a nursing liaison among the Inpatient Psychiatric Services, the Emergency Department and appropriate community resources. The Psychiatric Emergency Program also acts as Admission Co-ordinator to Magnolia House as designated and on weekends.
Qualifications
Education & Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Two (2) years’ recent related mental health and substance use experience including experience with patients in emergency, urgent care and/or medical/surgical areas, or an equivalent combination of education, training, and experience.
Knowledge, Skills & Abilities
Demonstrated ability to provide comprehensive head-to-toe nursing assessments and interventions related to common diagnosis and specific clinical situations relevant to the designated patient group (e.g. renal failure, wound care, diabetes, sepsis, intoxication, withdrawal).
Demonstrated ability to perform focused secondary assessments and integrate information from a variety of sources including diagnostics and develops a plan of care relevant to the designated patient group.
Demonstrated ability to anticipate and respond to potential complications and care issues relevant to the designated patient group.
Demonstrated ability in correct usage of equipment and maintains competency in working with Negative Pressure Wound Therapy (NPWT), oxygen delivery equipment, other care related pumps, catheters, tubes, suctions for the designated patient population.
Demonstrated skill in the management of patients with altered immune response, acute/chronic pain management, anticoagulation therapy, patients with problematic substance use/withdrawal challenges.
Demonstrated skill in Peripheral IV insertion, maintenance, blood collection, removal, and Tracheostomy care.

Two (2) years’ recent related experience providing treatment services to individuals with mental health issues, or an equivalent combination of education, training and experience.

Position works with older adults who are experiencing dementia or have major psychiatric disorders, concurrent disorders and/or problems normally associated with aging and are at risk due to a combination of psychiatric symptoms, complex medical/social factors or significant caregiver stress. Duties include psychogeriatric assessments, crisis intervention, case coordination, treatment planning, consultation/liaison, facilitating/co-facilitating individual/group sessions, individual counseling and education and support to promote integration and continuity across the care continuum. Consults with general practitioners, caregivers and family members and works with staff in continuing and residential care as well as hospitals as appropriate. Coordinates community resources and provides educational programs as required. Establishes and maintains relationships with, and consults with, clients, their families and colleagues to provide appropriate treatment services based on best practices and client need. Shares knowledge and expertise with colleagues via consultations, in-services and presentations. Acts as a clinical resource for nurses and other health care providers in the hospital and community. May provide direct care as well as outreach services.

Position works with older adults who are experiencing dementia or have major psychiatric disorders, concurrent disorders and/or problems normally associated with aging and are at risk due to a combination of psychiatric symptoms, complex medical/social factors or significant caregiver stress. Duties include psychogeriatric assessments, crisis intervention, case coordination, treatment planning, consultation/liaison, facilitating/co-facilitating individual/group sessions, individual counseling and education and support to promote integration and continuity across the care continuum. Consults with general practitioners, caregivers and family members and works with staff in continuing and residential care as well as hospitals as appropriate. Coordinates community resources and provides educational programs as required. Establishes and maintains relationships with, and consults with, clients, their families and colleagues to provide appropriate treatment services based on best practices and client need. Shares knowledge and expertise with colleagues via consultations, in-services and presentations. Acts as a clinical resource for nurses and other health care providers in the hospital and community. May provide direct care as well as outreach services.

Hilltop House is located in Squamish, BC, approximately one hour north of the Vancouver lower mainland and accessible on the scenic Sea to Sky highway. The area hosts an abundance of recreational activities such as mountain biking, hiking, rock climbing and kayaking, as well as a growing arts and culture scene. Hilltop House is an 86-bed long term care facility for seniors with complex needs. It provides home-like care for its permanent residents, as well as respite care, a community bathing program and adult day care for seniors.
Qualifications
Education & Experience:
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Completion of an approved gerontology course or an equivalent combination of education, training and experience.
Skills & Abilities:
Demonstrated ability in nursing practice related to residents who may have chronic diseases.
Demonstrated knowledge of pathophysiology of common medical conditions, aging changes, polypharmacy, treatment protocols, and nursing interventions related to common diagnoses and specific care situations related to care of adults living in Long Term Care (e.g. Stroke, Chest pain, Pneumonia; Pain management; Wound Care; Delirium; Dementia; Depression; other mental health conditions; Frail elderly residents with physical and/or cognitive impairments including Acquired Brain Injuries; Chronic illnesses such as COPD and CHF, Diabetes).
Demonstrated ability to provide nursing assessments & interventions related to common diagnoses and specific care situations relevant to residents (e.g. Respiratory symptoms such as Shortness of Breath; Pressure Ulcer (Braden Scale); Delirium (PRISME/CAMI); Dementia, (PIECES framework); Behavioral and Psychological Symptoms of Dementia (BPSD), GPA - Gentle Persuasion Approach; Dysphagia; Falls; Incontinence).
Demonstrated ability in recognizing physical adaptations required to accommodate the frail elderly and/or physically impaired resident population.
Demonstrated ability in End of Life care approaches, Embedding Palliative Approach in Care (EPAC) in long term care and systems of family dynamics and end-of-life issues. Proactively leads family/resident/Most Responsible Clinician discussions on goals of care and documents reflections of Medical Orders for Scope of Treatment.
Demonstrated ability in Chronic Disease Management by being able to anticipate and respond to potential complications and care issues relevant to the designated resident group.

Position Summary:
The Canadian Mental Health Association, Simcoe County Branch seeks a Registered Practical Nurse (RPN) provides client-centred, recovery-oriented care within a multidisciplinary mental health team, supporting individuals with complex mental health and substance use needs. The role involves conducting comprehensive assessments, administering and monitoring medications, and developing and implementing individualized care plans to promote stabilization, rehabilitation, and community integration. The RPN delivers direct nursing care, counselling, crisis intervention, and psychoeducation, while supporting daily living skills, health promotion, and relapse prevention. Working collaboratively with internal teams and external partners, the RPN coordinates care, advocates for client needs, and ensures continuity of services, while maintaining accurate documentation and adhering to professional, legal, and organizational standards.
What You'll Do:
Perform all aspects of nursing care within the professional scope of a RPN in accordance with standards of nursing established by the College of Nurses of Ontario.
Perform nursing and general duties as specified within the program’s model of care.
Perform as a member of a critically essential program with other multidisciplined clinicians.
Review and transcribe Doctor’s orders.
Responsible for monitoring blood work via Local Labs and CSAN (clozapine)
Conduct comprehensive physical and mental health assessments, including monitoring symptoms, behaviours, and response to treatment.
Administer, monitor, and document medications (including clozapine), ensuring compliance, completing medication reconciliation, and coordinating with prescribers, pharmacies, and lab services.

Develop, implement, and evaluate individualized recovery/OCAN care plans, coordinating interventions to support mental health stabilization, daily functioning, housing, and overall recovery goals.
Provide direct nursing care and client support, including counselling, psychoeducation, crisis intervention, and health promotion (e.g., lifestyle, wellness, medication education).
Deliver rehabilitation and skill-building support, including activities of daily living (ADLs), social functioning, and community integration.
Facilitate and lead therapeutic and psychoeducational groups as required.
Collaborate within an interdisciplinary team to deliver client-centred, recovery-oriented care, and consult with internal and external service providers to ensure continuity of care.

Required Qualifications:
A Registered Practical Nurse registered as a member in good standing with the College of Nurses of Ontario.
1-3 years experience in psychiatric nursing (community-based preferred).
Submission of a Vulnerable Sector Check (VSC), including a Criminal Record Check (CRC) in good standing.
Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
Fluency in French or another second language would be an asset.
Legally entitled to work in Canada.

Position Summary:
The Canadian Mental Health Association, Simcoe County Branch seeks a Registered Practical Nurse (RPN) provides client-centred, recovery-oriented care within a multidisciplinary mental health team, supporting individuals with complex mental health and substance use needs. The role involves conducting comprehensive assessments, administering and monitoring medications, and developing and implementing individualized care plans to promote stabilization, rehabilitation, and community integration. The RPN delivers direct nursing care, counselling, crisis intervention, and psychoeducation, while supporting daily living skills, health promotion, and relapse prevention. Working collaboratively with internal teams and external partners, the RPN coordinates care, advocates for client needs, and ensures continuity of services, while maintaining accurate documentation and adhering to professional, legal, and organizational standards.
What You'll Do:
Perform all aspects of nursing care within the professional scope of a RPN in accordance with standards of nursing established by the College of Nurses of Ontario.
Perform nursing and general duties as specified within the program’s model of care.
Perform as a member of a critically essential program with other multidisciplined clinicians.
Review and transcribe Doctor’s orders.
Responsible for monitoring blood work via Local Labs and CSAN (clozapine)
Conduct comprehensive physical and mental health assessments, including monitoring symptoms, behaviours, and response to treatment.
Administer, monitor, and document medications (including clozapine), ensuring compliance, completing medication reconciliation, and coordinating with prescribers, pharmacies, and lab services.
Monitor labs and diagnostic results (e.g., blood work), follow up on abnormalities, and communicate changes in client status to the clinical team and psychiatrists.
Develop, implement, and evaluate individualized recovery/OCAN care plans, coordinating interventions to support mental health stabilization, daily functioning, housing, and overall recovery goals.
Provide direct nursing care and client support, including counselling, psychoeducation, crisis intervention, and health promotion (e.g., lifestyle, wellness, medication education).
Deliver rehabilitation and skill-building support, including activities of daily living (ADLs), social functioning, and community integration.
Facilitate and lead therapeutic and psychoeducational groups as required.
Collaborate within an interdisciplinary team to deliver client-centred, recovery-oriented care, and consult with internal and external service providers to ensure continuity of care.
Expected to function within an interdisciplinary role that includes therapeutic, vocational, educational, and nursing interventions
Provide education and support to families and natural supports.
Support clients in obtaining and maintaining housing and community services, including completing assessments (e.g., SPDAT) and providing advocacy.
Participate in risk management, care planning, and team processes, including reviewing reports, documenting care, and contributing to clinical decision-making.
Provide crisis response support during working hours and assist with stabilization planning.
Responsible for transporting clients in the community and, on occasion, outside of the geographical area.
Perform other duties as assigned by the Program Manager or organizational leadership.
Required Qualifications:
A Registered Practical Nurse registered as a member in good standing with the College of Nurses of Ontario.
1-3 years experience in psychiatric nursing (community-based preferred).
Submission of a Vulnerable Sector Check (VSC), including a Criminal Record Check (CRC) in good standing.
Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
Fluency in French or another second language would be an asset.

Position works as part of an interdisciplinary team to provide triage and outreach services that are inclusionary and flexible for individuals with mental illness, substance use and/or chronic medical conditions.
Conducts mental and physical status assessments and assesses the client’s physiological, psychological, sociocultural and spiritual needs to determine priority of care requirements based on client needs, availability of resources and best practices. Provides counseling and assists client with reducing repeated contact with and use of acute and community healthcare services/programs/resources. Assesses high risk situations and provides crisis intervention.
Collaborates with the interdisciplinary team to develop, implement and adjust the client’s care plan to meet the client’s need for integrated services and continuity of care across the continuum. Facilitates the movement of clients between acute care, community agency support site and designated health centre. Participates in case conferences/meetings with members of the interdisciplinary team. Assists clients with attendance to needed services such as medical appointments.
Practices in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards (professional & practice) and code of ethics for registered nurses or registered psychiatric nurses. Practices within the scope of practice as set by the organization, as per VCH policies and guidelines/clinical practice documents and within own competence. Assertive Community Treatment (ACT)
ACT teams bring health care and life-skills support to clients; all of whom would have difficulty arranging and keeping appointments with traditional office-based health care practitioners. ACT uses a trauma informed approach to promote recovery through community-based treatment and rehabilitation. The ACT team strives to reduce the number of contacts between police and their clients with severe and persistent mental health conditions and substance use concerns.
Qualifications
Education and Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Two (2) years’ recent related experience working with individuals with mental health and substance use issues, or an equivalent combination of education, training and experience.

Position works as part of an interdisciplinary team to provide triage and outreach services that are inclusionary and flexible for individuals with mental illness, substance use and/or chronic medical conditions.
Conducts mental and physical status assessments and assesses the client’s physiological, psychological, sociocultural and spiritual needs to determine priority of care requirements based on client needs, availability of resources and best practices. Provides counseling and assists client with reducing repeated contact with and use of acute and community healthcare services/programs/resources. Assesses high risk situations and provides crisis intervention.
Collaborates with the interdisciplinary team to develop, implement and adjust the client’s care plan to meet the client’s need for integrated services and continuity of care across the continuum. Facilitates the movement of clients between acute care, community agency support site and designated health centre. Participates in case conferences/meetings with members of the interdisciplinary team. Assists clients with attendance to needed services such as medical appointments.
Practices in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards (professional & practice) and code of ethics for registered nurses or registered psychiatric nurses. Assertive Community Treatment (ACT)
ACT teams bring health care and life-skills support to clients; all of whom would have difficulty arranging and keeping appointments with traditional office-based health care practitioners. ACT uses a trauma informed approach to promote recovery through community-based treatment and rehabilitation. The ACT team strives to reduce the number of contacts between police and their clients with severe and persistent mental health conditions and substance use concerns.

Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Two (2) years’ recent related experience working with individuals with mental health and substance use issues, or an equivalent combination of education, training and experience.

The Adult Community Mental Health team (located at the HOpe Centre) provides specialized, community-based services for adults living with serious or persistent mental illness, as well as individuals who may be in the early stages of developing a mental health disorder. Clients are supported through inclusionary, flexible care, with services tailored to individual needs and delivered through a combination of in-office appointments, home visits, and community outreach.

The HOpe Centre on the Lions Gate Hospital campus is the focal point for mental health services on the North Shore. What you’ll do
Provide direct health care services to children and youth presenting with a wide variety and complexity of mental health presentations. Perform physical, psychosocial and mental health assessments, collaborate with physicians and allied health as required, administer prescribed treatments and medications according to policy.
Observe, monitor, evaluate and documents patient progress, symptoms and behavioral changes. Perform a variety of health care interventions, utilizing nursing skills and medical equipment including therapeutic interventions using a variety of modalities.
Assess the patient’s needs in collaboration with the emergency team during the intake procedure to determine patient disposition. Develop care and treatment plans with final dispositions authorized by the emergency department physician and/or consulting psychiatrist. Provide general and crisis counseling to patients and their families including brief crisis intervention, referrals to community resources, or emergency department social workers. Initiate and facilitate discharge planning and referrals.
Support the process of admission and transfer to inpatient units by supporting the needs of the patient and their family, providing information to the receiving unit and facilitating transition by sharing emergency treatment outcomes and care plans to the inpatient unit.
Work with team members to develop educational resources that may be used with patients/families for health teaching.
What you bring

What you’ll do
Provide direct health care services to children and youth presenting with a wide variety and complexity of mental health presentations. Perform physical, psychosocial and mental health assessments, collaborate with physicians and allied health as required, administer prescribed treatments and medications according to policy.
Observe, monitor, evaluate and documents patient progress, symptoms and behavioral changes. Perform a variety of health care interventions, utilizing nursing skills and medical equipment including therapeutic interventions using a variety of modalities.
Assess the patient’s needs in collaboration with the emergency team during the intake procedure to determine patient disposition. Develop care and treatment plans with final dispositions authorized by the emergency department physician and/or consulting psychiatrist. Provide general and crisis counseling to patients and their families including brief crisis intervention, referrals to community resources, or emergency department social workers. Initiate and facilitate discharge planning and referrals.
Support the process of admission and transfer to inpatient units by supporting the needs of the patient and their family, providing information to the receiving unit and facilitating transition by sharing emergency treatment outcomes and care plans to the inpatient unit.
Work with team members to develop educational resources that may be used with patients/families for health teaching.
What you bring
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses & Midwives (BCCNM).
One (1) year recent related experience working with children, adolescents and families in a child/adolescent mental health or inpatient setting, or an equivalent combination of education, training and experience.
