Temporary Contract Position - Internal/External Posting

 

One (1) Full-Time Contract Position (37.5 hours per week)

Street Health is a community-based agency providing innovative physical and mental health programs and advocacy to homeless and insecurely housed individuals in downtown Toronto for over 30 years.  Street Health, a not-for-profit organization located within the downtown-east neighborhood of Toronto, works to improve the health and wellbeing of people who are homeless and under-housed by addressing the social determinants of health. Our organization provides services in the areas of innovative nursing, mental health, harm reduction, Overdose prevention, Safer Supply, outreach, identification replacement programs, and through advocacy, research, and education.

 

This position is for a contract position to cover a maternity leave of absence. Please be aware that the contract end date may be extended or may be adjusted to an earlier end date should the employee on leave return earlier than expected.

 

Responsibilities of the position include, but may not be limited to:

 

  • Provide nursing care to clients while maintaining up-to-date and accurate charts;
  • Participate in and provide referrals to community agencies and health care practitioners; accompany clients to appointments when needed; support clients in from an advocacy perspective;
  • Participate, initiate and support systemic advocacy, grassroots organizing, awareness raising and public education for positive social change via involvement in community-based committees and networks;
  • Contribute to program and policy development;
  • Support students and volunteers; participate in and provide consultation to team members through case management;
  • Provide awareness & education to students, staff members, community health care institutions and community agencies regarding the communities we serve;
  • Support and provide access for clients to low-barrier services at Street Health;
  • Assist in the collection and documentation of statistical data;
  • Provide services in the Overdose Prevention Site (OPS) when needed;
  • Promote the mission, vision and values of Street Health; and
  • All other duties as may be assigned within the scope of this position.

 

Qualifications

 

  • BScN or BN.
  • Current registration as a Registered Nurse, in good standing with the College of Nurses of Ontario.
  • Minimum two years’ general nursing experience; competent clinical/assessment skills.
  • Relevant experience working within a community-based, not-for-profit organization.
  • Demonstrated knowledge of mental health disorders, treatment modalities and social determinants of health.
  • Experience advocating for clients, individually and at a systemic level.
  • Strong clinical assessment and documentation skills.
  • Strong oral and written communication skills; strong computer skills and experience using EMR software.
  • Experience working with marginalized groups and strong understanding of systems and issues affecting individuals who experience mental health disorders and homelessness is an asset.
  • Experience working within a team/organization harm reduction approach is an asset.
  • Certification in Advanced Nursing Footcare, or interest and willingness to attend training.
  • Ability to carry out duties independently while maintaining accountability to agency.
  • Valid CPR certification or ability to obtain certification within the first 3 months of employment.
  • Successful police services Vulnerable Sector Screening Check.
