Nurse Practitioner - Permanent Full Time
The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. Position Summary:
Reporting to the Patient Care Manager, Toronto Grace Health Centre is currently seeking a Nurse Practitioner at our Scarborough location. The Nurse Practitioner is an active member of the interdisciplinary team that also includes a social worker, occupational therapist, and physiotherapist. The NP has clinical and leadership expertise in the provision of quality care for diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting. They will act as a leader and mentor for the professional nurses, allied health professionals, administrative and other supportive staff on the unit.
Responsibilities:
Provide client and family-centered care independently and in collaboration with other team members.
Demonstrate all the Advanced Nursing Practice Competencies outlined by the Canadian Nurses Association including clinical practice, research, leadership, consultation, and collaboration.
Conduct the patient care health assessment and diagnosis, therapeutic management, and health promotion and prevention of illness and injury.
Work in collaboration with the interdisciplinary team in a shared-care model of practice providing cross coverage.
Liaise regularly with external health organizations.
Support best practice in seniors’ care and other health care providers where appropriate.
Consult on patient/client care, providing advice, expertise, and consultation services.
Qualifications:
Currently registered with the CNO as a Registered Nurse in the Extended Class.
Currently have RNAO or NPAO membership with LAP or equivalent.
Additional post graduate education or certification in Mental Health and Addictions services / management related areas is a strong asset.
Minimum Three (3) years of relevant clinical experience, including experience working with clients with complex mental health illnesses.
Experience working with persons with Dementia or cognitive impairment.
Successfully completed a Primary Health Care Nurse Practitioner Program in Ontario or equivalent and hold a master’s degree in nursing with a Primary Health Care Nurse Practitioner Program (family all ages).
Your clinical focus will be with chronic disease management, geriatric nursing care, mental health, and addictions services where there is an emphasis on collaborative practice within an inter-professional milieu.
Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, push and pulling.
You have demonstrated clinical leadership skills and a commitment to continued participation in research.
Must have received at least two doses of a COVID-19 vaccine approved by Health Canada prior to start date and submit evidence of such. Determines priority of care requirements based on client needs, availability of resources and best practices. Develops strengths-based policies and procedures. Oversees and coordinates the delivery of crisis intervention services/supports/daily activities of the team, participates in case planning; and provides leadership/supervision to designated staff. This position is part of the new Indigenous Crisis Response Team (ICRT) at VCH. The ICRT provides a non-police crisis response outreach services to individuals in the inner city who are experiencing a mental health and/or substance use crisis that cannot be resolved by their regular care provider or alternate crisis services. ICRT is a voluntary, non-police healthcare service that leads with cultural safety and sees individuals in encampments, shelters, or in their homes.
QUALIFICATIONS
Education & Experience
Current full registration with the applicable professional College or professional association and Degree in a recognized health sciences profession supplemented
Five (5) years’ recent, related experience working in Indigenous-led or First Nations organizations providing mental health and substance use clinical services in community-based settings for designated program or an equivalent combination of education, training and experience.
Valid BC Driver’s License.

The Director, Quality, Professional Practice and Patient Safety is currently seeking two full-time Clinical Excellence Consultants. REQUIREMENTS/QUALIFICATIONS:
Current registration as a Registered Nurse (General Class) in good standing with the College of Nurses of Ontario.
A Master’s Degree in Nursing or Health Administration is a requirement, although other pertinent Master’s Degrees may also be considered.
A minimum of 5 years of clinical experience in mental health and/or addiction services within an acute care community hospital or tertiary hospital setting, including at least 2 years in an inpatient setting. Experience in a clinical leadership role is preferred.
Education or certification in continuous quality improvement is preferred. Education or certification in change management is preferred.
Possession of current CNA certification in Psychiatric and Mental Health Nursing (CPMHN(c)) is preferred
Ability to work independently, be self-directed, and manage competing priorities effectively
Capability to integrate research and evidence-based practice into clinical approaches Excellent written and verbal communication and presentation skills
Experience in adult education/curriculum development and delivery with aptitude in mentorship and coaching

The Director, Quality, Professional Practice and Patient Safety is currently seeking two full-time Clinical Excellence Consultants. This position is pivotal in Waypoint's commitment to delivering exceptional care, enhancing professional practice, and promoting a culture of continuous improvement and innovation. This role integrates advanced knowledge and a proven record in implementing and sustaining evidence-based practices in the clinical areas of Mental Health and Addictions/Geriatric Mental Health/Forensic Mental Health, combined with robust leadership skills. The incumbents will drive clinical excellence of care across hospital services by developing, implementing, and evaluating program changes to enhance care quality and accessibility. Together with the clinical managers, the incumbent will support interdisciplinary teams in providing evidence-based, patient-centered care, significantly enhance overall staff clinical performance, and ensure strict adherence to the standards of care established by their respective professional colleges.

KEY ACCOUNTABILITIES:
Act as an expert and resource for clinical teams in the implementation and sustainability of evidence-based practices for patient population with complex care needs
Collaborates effectively with colleagues at all organizational levels to enhance patient safety and manage risks.
Works closely with program leadership to ensure the implementation of evidence-based practices to improve patient outcomes.
Assumes a leadership role within the program and interdisciplinary team to develop service delivery models, collaborating with program leadership to enhance the experiences of patients and their families.
Collaborates with clinical managers in assigned areas to ensure that their direct reports function within their discipline's full scope of practice and reinforcing professional practice standards
Collaborate with clinical managers to conduct investigation interviews concerning professional practice matters involving regulated staff; assist in drafting reports for submission to the relevant regulatory colleges
Works with other leadership team members to identify strategies for enhancing staff knowledge and skills in inter-professional teamwork and quality care provision.
Leads the Waypoint-wide implementation of priority quality/safety practice innovations and high-risk care planning standards.
