The North Hamilton Community Health Centre was established in 1987 is located in the North End of Hamilton. The services and programs of Health Centre serves people residing in the immediate neighbourhood that require primary care and allied care services but also expand to include diabetic foot care and outreach services across the Hamilton Niagara Haldimand Brant Local Health Integrated Network (LHIN). Clients of the Health Centre come from diverse backgrounds including individuals with complex chronic care needs, seniors and newcomers to Canada. The Health Centre offers programs for seniors, children and youth.
The Health Centre also offers the Pathways to Education program to students in high school - Pathways is a stay in school program supported by mentoring, tutoring, advocacy and financial support to improve the high school graduation rate in the North End and neighbouring community.

As a Community Health Centre we understand that health of individuals and communities is impacted by the social determinant of health and we strive for improvements in social supports and programs that will have a positive impact on health outcomes for individuals and communities.
