VP Finance, Corporate Services, Redevelopment and CFO Located approximately 100 kilometres east of Toronto, NHH delivers a broad range of acute, post-acute, outpatient and diagnostic services. Acute services include emergency and intensive care, medical/surgical care, obstetrical care and palliative care. Post-acute specialty services (PASS) include restorative care and rehabilitation. Mental health care, cancer and supportive care, dialysis and other ambulatory care clinics are offered on an outpatient basis through partnerships with regional centres and nearby specialists. NHH offers a full range of diagnostic services, including magnetic resonance imaging (MRI), computed tomography (CT) and mammography. The hospital serves the catchment area of west Northumberland County. A mixed urban and rural population of approximately 67,000 residents, west Northumberland comprises the Town of Cobourg, the Municipality of Port Hope, Alderville First Nation and the townships of Hamilton, Cramahe and Alnwick/Haldimand. YOUR OPPORTUNITY TO MAKE A DIFFERENCE Reporting to the CEO, the VP Finance, Corporate Services, Redevelopment and CFO is a strategic partner responsible for Hospital’s Resources and Financial Services. As a key member of the Hospitals Senior Management Team, you will have a significant leadership role in developing strategies that result in the efficient utilization of financial resources while supporting patient care and our agenda for growth. Additional accountabilities for the role will include leadership of the Capital Redevelopment, Facilities, Security and Procurement portfolios. Translating the hospitals’ strategic directions into outcomes, the VP will work with the leadership team to ensure that our organization is positioned to support the growth in our community, while enhancing quality of care delivered, realizing a positive workplace culture and building partnerships internally and externally. As the VP, you will demonstrate collaborative leadership, working closely with members of the senior management team while supporting and mentoring those reporting directly to you. QUALIFICATIONS Management: Substantive leadership experience in an executive capacity demonstrated through progressive experience ideally focused in a hospital or health services setting. Strategic Acumen: Ability to think critically, to analyze situations, understand environmental and political implications and devise strategies and tactics to help us attain our goals and objectives. Finance: Expert level knowledge and experience with current practices related to budget planning, accounting, internal controls, financial reporting and capital planning in a health services setting. People Leadership: Experience and ability to provide exceptional leadership to staff and to build enthusiastic, high performing staff Business Transformation: Ability to demonstrate customer service excellence across the organization through effective delivery of innovative programs and services community in time-sensitive and changing environments. Program Evaluation: Experience in evaluating organizational improvement initiatives using evidence-based practice. Engagement and Partnership: Experience engaging others, building consensus, delivering strategies, common solutions and results with other service providers. Capital Redevelopment: A solid understanding of, and ideally demonstrated leadership experience as it pertains to capital redevelopment in a healthcare setting, Healthcare: Thorough understanding of the Ontario health reform agenda including funding models and the continuum of health care services. Communications: Exceptional political acuity; ability to communicate effectively and address concerns with tact and diplomacy; excellent written, verbal and presentation skills. Education: Bachelor’s degree with a major in finance, business or a related field and a CPA in good standing; MBA or master’s level education in a relevant field is an asset. Nurse Practitioner -Transition to Adult Care Program

The Nurse Practitioner will have a unique opportunity to work alongside an interdisciplinary team and co-create a new model of care that will focus on supporting transition for youth with chronic, complex medical conditions, rare disease and social complexity to adult services. The Transition to Adult Care Program is an initiative that seeks to support youth with complex, chronic illness, who often experience challenges with transitioning from pediatric to adult care. As SickKids moves towards a more individualized model of care powered by Precision Child Health, we have a duty to design and implement solutions that bridge Unprecedented Outcomes from childhood to adulthood. The Nurse Practitioner will play an instrumental role in coordinating care, empowering youth and enabling providers across the continuum of care to assume collaborative care management of youth as they transition to the adult healthcare system, in the community and primary care settings. This is an exciting opportunity to forge a new role within the health care team and innovate to find optimal solutions. We are looking for someone who is extremely motivated, flexible, and able to act as a change agent to fulfill this role. Here's What You'll Get To Do: Assist in the development of a new model of care that supports youth with chronic illness, rare disease and social complexity transition to adult services. Perform detailed risk and needs assessments and advocate for youth needs. Participate in intensive care coordination for time limited period to ensure adequate care through transition. Assume a leadership role in influencing clinical advanced nursing practice and the provision of care. Integrate research into practice. Co-develop care plans with youth to reflect transition goals and enable connection to adult primary care providers (PCPs) and specialists. Assess youth transfer needs (e.g. readiness, preparedness, adherence). Empower youth through the development of self-management skills and motivational interviewing. Ongoing partnership and collaboration with primary care practitioners, adult care specialists to ensure a seamless transition. Support referrals and joint appointments between pediatric and adult subspecialists (virtually or in person, as appropriate) Assist with funding applications in collaboration with other providers (eg: social work) and ensure families receive supports they are entitled to. Troubleshoot transition related issues. Here's What You'll Need: Essential Requirements Master's of Nursing or equivalent with a clinical focus. Completion of a Nurse Practitioner program (within MN or post-master certificate) Registration with the College of Nurses of Ontario, within the RN (Extended Class) Minimum of 2 years of clinical experience working with adolescents and/or young adults and navigating youth related issues Working knowledge/experience in Primary and/or Adult care systems Ability to navigate heath care resources and manage partnerships in both hospital and community settings. Demonstrated initiative and ability to be self-directed as well as work as part of an interdisciplinary team. Ability to work effectively with youth who are undergoing stressful life experiences with a trauma-informed, culturally appropriate lens. Excellent interpersonal, communication, organizational, time management skills and attention to detail Demonstrated ability to use critical thinking and creative problem-solving skills. Strong advocacy skills and motivation to pursue systems level change. Commitment to practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
