Respond to the needs of the Consult Liaison Service the CLN supports a model of collaborative care through coordination of referrals to the Consult Liaison service and providing case management, clinical consultation to nursing and medical staff, liaison with other involved professionals (i.e. medical Psychology), psychiatric care, discharge planning, follow-up and liaising with community care providers.
Initiate assessments by gathering information, evaluating information, consults in creating/planning and implementing care plan. Participates as a member of the health care team in patient case conferences for patient reviews, treatment and discharges. Develop teaching plans for patients, and families and health care providers in the hospital and community settings about Mental Health Disorders and provides self-harm consultation to other disciplines (i.e. social work, medicine, nursing). Provide continuing education for the nursing staff including formally advising nursing staff in nursing practice, providing clinical instruction, supporting staff training and development.
Actively participates in quality of care measures, and support pediatric somatization research and other research opportunities (e.g. epilepsy). Participate in nursing research and other interdisciplinary projects through reviewing current research, collecting data, analysing information and identifying issues/practices which require research, and evaluating the implementation of current theory into practise.
Act as an educational resource for leading and best practices, and is actively involved in provincial knowledge translation initiatives. What you’ll do

The Substance Use Program within the Division of Adolescent Medicine at SickKids is establishing a new initiative that will address critical gaps in addiction services for youth aged 12-25 in the Greater Toronto Area. The primary focus is to provide a structured virtual consultation program for practitioners who serve youth with addiction and substance use health concerns, provide training in evidence-based practices, and enhance service delivery and bridging the gap between youth services and adult addiction services.
The program will also provide training in evidence-based practices through a variety of learning formats. Overall, the program's goals are to enhance the quality of care for youth with substance use and associated health concerns, as well as help to bridge the gap between youth and adult addiction services.
Here's What You'll Get To Do:
Design & Project Development: Work with the project team to develop the consultation service model (such as mapping out intake criteria and mechanics of consultation service)
Provide virtual consultation to health care practitioners working with youth aged 12-25 in a variety of settings through an online referral and booking system
Collaborate with local partners developing a clinical practice model and participate in knowledge exchange activities.
Contribute as a member of the Substance Use Program in the Division of Adolescent Medicine and clinical consultations within the hospital.
Collaborate with colleagues across the hospital to build capacity for substance use care through dissemination of best practices and evidence informed care.
Develop relationships with various organizations who see youth and/or provide youth addiction services in the GTA
Liaise with community partners and promote engagement across the sector.
Participate in the development, delivery and evaluation of education around evidence informed approaches to working with youth with substance use and substance use disorders.
Here's What You'll Need:
Master of Nursing or equivalent with a clinical focus.
Completion of a Nurse Practitioner certificate.
Registration with the College of Nurses of Ontario, within the RN (Extended Class).
Completion of education required by the College of Nurses of Ontario on prescribing controlled substances.
Assets:
A minimum of three years of clinical nursing experience in adolescent health, and/or Substance Use and Mental Health.
Ability to interpret, incorporate and utilize evidence in the context of unique patient populations with varying levels of acuity and complexity.
Knowledge and understanding of developmentally and trauma informed care for children and adolescents.
Excellence in clinical nursing skills focused on medical monitoring, monitoring and follow-up of both medical and mental health related issues; demonstrated ability to collaborate with interprofessional colleagues around these duties and tasks.

The Vancouver Adult MHSU team supports adults 19 years of age and older with moderate to severe mental health, substance use, and concurrent mental health and substance use disorders or conditions. This program is designed to be neighbourhood based and works as an interdisciplinary team to encourage collaboration and partnerships with local community services, including family physicians and other care providers.
As a Mental Health Nurse with VCH, you will:
Provide individualized direct nursing care and brokerage services to a wide range of clients with severe and persistent mental illness, concurrent disorders and/or dementia and be responsible for the full range of clinical practice and the client-centred rehabilitation program by working independently and as a member of a team.
Enhance the quality of life of the client/family.

Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Diploma in Nursing or Psychiatric Nursing plus two (2) years’ recent related experience working with individuals with severe and persistent psychiatric illness or an equivalent combination of education, training and experience.
Valid BC Driver’s license and local area travel may require the use of a vehicle, as required.

Venture Crisis Home
Venture is a short-term crisis intervention and stabilization program for mental health and substance use clients as an alternative to hospitalization located in Vancouver Community. This 24-bed licensed facility offers 24-hour care and facilitates clients to successfully transition back to community supports. Venture has 24 hour nursing and healthcare worker staffing, day time psychiatry and GP and recreational therapy.
As a Mental Health Nurse with VCH, you will:
Provide individualized direct nursing care and brokerage services to a wide range of clients with severe and persistent mental illness, concurrent disorders and/or dementia and be responsible for the full range of clinical practice and the client-centred rehabilitation program by working independently and as a member of a team.
Enhance the quality of life of the client/family.
Perform physical and psychosocial rehabilitation assessments.
Consult, confer and collaborate with other health care givers and family members to develop, implement, evaluate and modify individualized strength-based treatment plan.
Establish, maintain and enhance therapeutic relationships based on respect.
Administer prescribed medicines, monitor client response and demonstrate and provide ongoing information to the client/family on activities of daily living, use of medication and side-effects and integration of illness to their lifestyle.
Prepare and maintain client records according to established standards.
Participate in team meetings, engage in team planning and quality improvement activities and maintain and update your own knowledge.
Qualifications
Education & Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Diploma in Nursing or Psychiatric Nursing.
Two (2) years’ recent related experience working with individuals with severe and persistent psychiatric illness or an equivalent combination of education, training and experience.
Valid BC Driver’s license and local area travel may require the use of a vehicle, as required.
Knowledge & Abilities
Knowledge of the principles and practices of a client and family centered recovery model in mental illness.
Knowledge of concurrent disorders.
Knowledge of Psychosocial Rehabilitation methodology.
Knowledge of the Mental Health Act as it applies to the rights and obligations of clients and staff.
Knowledge of community, social and mental health resources.
Ability to identify emotional, social, or environmental barriers and intervene to eliminate stigma.
Ability to effectively manage conflict situations and aggressive behaviour and use non-violent crisis intervention and behavioural management techniques.
Ability to work both independently and collaboratively as a member of an interdisciplinary team.
Ability to communicate effectively, both verbally and in writing with clients, families, team members, other groups and community agency personnel.
Knowledge of trauma-informed care.
Demonstrated ability in nursing practice related to assessment of behaviour and to assisting clients to regain or improve their coping abilities, and to prevent further disability.
Demonstrated ability to lead and co-lead group sessions.
Demonstrated ability to plan and implement care plans.
Demonstrated ability to assess client responses to care, and to respond appropriately.
Demonstrated ability to adjust to unexpected events, problem-solve and set priorities.
Demonstrated ability to participate in the development and delivery of educational programs and materials for clients and families based on education theories, principles and best practices.
Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
Demonstrated ability to counsel and teach clients and their families.

Demonstrated ability to provide leadership through initiative and independence.
