The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 2 sites – the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). POSITION SUMMARY
Reporting to the President & CEO the Director, Fundraising is an experienced, result-oriented self-starter. This role leads and is responsible for the development and execution of a strategic plan in partnership with colleagues within the Hospital and Board of Trustees. The Director will be accountable for the direction of the Hospital’s planned giving efforts, resulting in leading-edge fundraising practices including but not limited to online and other modes of donor events, telemarketing, direct mail, surveys, and a yearly fundraising and awareness event.
As a senior leader in the organization, this individual will be an influential member of the Hospital’s leadership team and help set strategic direction for donor activities.
This is an opportunity to build upon an existing program while bringing innovation, best practices, consistency, and sustainability. As the lead advocate for the program, the Director will work closely with the Social Media and Fundraising Coordinator, leadership, stakeholders and closely with colleagues that inspire donor support and build awareness.

The Salvation Army Toronto Grace Health Centre (TGHC) is a mission of The Salvation Army. Toronto Grace is a 150-bed facility located in the heart of downtown Toronto, and is renowned for the compassion, clinical excellence and patient-centred care we provide. The heart of patient-centred care is recognizing that each person has individualized priorities and goals, and each care plan reflects this. Through understanding and appreciating the complexity of day-to-day life, Toronto Grace provides care for individuals with multifaceted chronic diseases who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care and Integrated Transitional Services.
Our Complex Continuing Care (CCC) program serves persons with complex and multiple chronic diseases and functional disabilities who require a wide range of therapeutic services and clinical specialties. The CCC program provides medical management, skilled registered nursing care, as well as therapy, recreation and spiritual services. The program offers care of the individual following a restorative and preventative approach. The Grace offers comprehensive services to support individuals so they may enjoy their community, family and friends.
Post Acute Care Rehabilitation (PACR) Program provides quality and integrated care for patients: who are recovering from orthopedic surgery, hip and other fractures; those with medically complex conditions and post-stroke complications affecting motor skills; and those who have suffered stroke and neurological issues. Our dedicated team of skilled interprofessional health team that includes of Physicians, skilled Rehab Nurses (registered and practical), Physiotherapists, Occupational Therapists, Speech Language Pathologist, Communication Disorders’ Assistant, Recreation Therapist, Social Worker, Chaplain and Clinical Dietitian. A central therapy room is located on the main floor to support an integrated therapy plan and rehabilitation. Persons accepted into PACR look forward to returning home and/or back into the community.
Palliative Care provides comprehensive end-of-life care to support the person and family at their end of life journey. Our holistic approach to care includes a skilled interdisciplinary team to meet the individual’s physical, psychosocial, spiritual and cultural needs. Persons in the community who wish to remain at home for as long as possible can be rapidly admitted to the Palliative Care Unit if needed.
Integrated Transitional Services is a new program which is located at The Salvation Army Toronto Harbour Light Ministries, and provides support for ALC patients with bariatric requirements and patients with mental health and addiction requirements who are medically stable and have acquired a level of independence but continue to require a level of support before they can transition into the community.
