We are actively recruiting for a Clinical Nurse Specialist, Permanent Full-Time

The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional Care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). Position Summary

The Clinical Nurse Specialist (CNS) is an advanced practice registered nurse.  As an integral member of the inter-professional team you will be accountable for the delivery and provision of patient centered care and provide expertise in the specialty area of both invasive mechanical ventilation as well as non-invasive respiratory support.  The CNS  will be responsible for assuming a leadership role in coordinating patient care within the healthcare facility; assessing risks and complications; monitoring outcomes; providing direct therapeutic interventions ensuring quality patient care outcomes based on best practice guidelines. The CNS will also be responsible in leading quality improvement initiatives and support others to use best practice guidelines.

Qualifications

  • Current registration with the College of nurses of Ontario
  • Successful completion of BScN: Master’s degree preferred.
  • Demonstrate membership within a professional association and/or professional interest groups related to practice preferred.
  • Current Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certification.
  • Client service oriented, with the ability to effectively work within a framework that supports diversity, inclusiveness, and equity.
  • Demonstrate respect and support for diversity within and among our patients, families, and teams.
  • Strong interpersonal and communication skills.
  • Excellent organization and time management skills.
  • Demonstrated effective problem solving and critical thinking skills.
