Perform nursing and general duties as specified within the ACTT model of care.
Complete physical health assessments to thoroughly determine the health status of clients as well as any medical conditions present, and ensure appropriate treatments, follow-up, and support are provided.
Participates in a multi-disciplinary consultive, client-centred recovery orientated specialized mental health treatment/medical service delivery model.
Provides the majority of the treatment, rehabilitation and support services, in addition to clinical & psychosocial rehabilitation and recovery for persons who have the most serious mental illness, have severe symptoms and functional impairments, and have not benefited from traditional outpatient programs.
The ACT team is mobile and delivers majority of treatment, rehabilitation, and assertive outreach in the community. 75% or more of the services are provided outside of the program offices.
Responsible for assessing and documenting the client’s mental illness symptoms and behaviors and response to medication and shall monitor for medication side effects. Report clinical assessments to team psychiatrist.
Responsible for delivery, observation, monitoring of compliance, and documentation of medication.
Review and transcribe Doctor’s orders.
Responsible for monitoring blood work via Local Labs and CSAN (clozapine)
Communicate with client pharmacies and perform as a collaborative member with Medisystems pharmacy.
Accountable and timely reporting to appropriate team clinicals/psychiatrists of any concerns or changes in mental status.
Daily review of transfer of accountability report, risk management interventions and care planning
Provide the team, clients, families, and others with specialized consultation and education related to nursing issues.
Develop, implement, and document, OCAN/Recovery Plans in conjunction with clients and other ACTT members.
Creation and facilitation of therapeutic and skill building groups.
Coordinate and monitor the activities of the clients’ individualized recovery plans, including but not limited to: psychiatric illness or symptom reduction, housing activities of daily living, daily structured employment and family and social relationships.
Provide counselling to assist clients in accomplishing their goals and managing their symptoms/day-to-day stressors.
Consult with other service providers to promote continuity of care for the clients.
Provide on-call crisis intervention to ACTT clients during regular work hours (and during after work hours on a rotational basis).
Provide support/education to clients’ families and social support networks.
Administer and monitor medications as per doctor’s orders. Responsible for packaging/checking medication for safe administration by non-nursing staff.
Assist clients with their ADL’s such as budgeting, shopping, meal planning/preparation, housekeeping, use of transportation, personal grooming, laundry, keeping appointments, etc
REQUIRED QUALIFICATIONS:
Minimum three (3) years’ experience in psychiatric nursing (community-based preferred).
A Registered Practical Nurse registered as a member in good standing with the College of Nurses of Ontario.
Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
Fluency in French or another second language would be an asset.
Legally entitled to work in Canada.
SALARY SCALE:

JOB SUMMARY: As a member of an interdisciplinary team, you will be expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. Identifying changes in appearance/behaviors, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviors.
Participating in service planning and development of personal goals for clients to transition to their permanent discharge location.
Providing support with behaviors arising from cognitive issues, intervening in conflicts between residents, and contacting appropriate emergency services in crisis situations.
Maintaining all records of service and ensuring supplies and equipment are readily available.
Liaising with physicians, pharmacists and other health care providers regarding client care.
Advising, coaching and supporting staff regarding medical/healthcare questions and concerns.
WHAT YOU WILL BRING Must-Haves
Current registration with the College of Nurses of Ontario and able to practice without restrictions.
Medication Administration Certification or completion of an RPN education preparation program that includes medication administration.
Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioral challenges who have been longer term hospital patients.
Knowledge and proven experience working with clients in a PSR (Psychosocial Rehabilitation) approach.
Knowledge of social services and community services for adults and seniors.
Knowledge of and experience in delivering direct services in accordance with psychosocial ‘recovery’ principles.
Ability to organize and work with individuals and groups in the areas of assessment, problem solving and crisis management; Ability to interact with clients in a positive and supportive manner.
Physically able to perform demanding body mechanics (i.e. lifting, transferring, Hoyer, etc.).
First Aid and CPR certification.
Good oral and written skills.
Excellent computer skills.
Ability to work effectively in a team environment.

The Licensed Practical Nurse performs a variety of nursing procedures and provides personal care to patients in areas such as Radiation Therapy, Systemic Therapy and In-Patient Nursing units. What you’ll do
Prepare patients for examination by performing duties such as escorting patients to the examining room, weighing patients, assisting them with undressing and providing comfort measures.
Prepare examining rooms for patient consultations and examinations by performing duties such as checking and laying out clean instruments, stocking rooms with required supplies, cleaning and making up room for next patient and collecting used instruments and sending to central processing for sterilization.
Assist nursing and medical staff with physical examinations by performing duties such as handing and receiving instruments and supplies, receiving and preparing specimens through methods such as levelling, attaching completed requisitions and placing in receptacles and/or delivering to the laboratory.
Perform a variety of nursing procedures such as catheterizations and sterile dressings by performing duties such as applying and changing sterile or non-sterile dressings, colostomy care and irrigation, taking/recording blood pressure and/or rectal temperature, administering suppositories and enemas, taking temperatures, pulse and/or respiration. Record observations and report problems and/or changes to designated staff.
Perform a variety of clerical duties such as organizing appointment lists, films and reports for clinic and ensuring charts are returned for order processing after examination, answering telephones, filing and typing utilizing a variety of software packages.
Maintains instruments and equipment such as nasopharyngoscopes and ophthalmoscopes by performing duties such as cleaning through methods such as operating steris system, checking function, reporting malfunctioning equipment to Leader for action and storing instruments and equipment ready for next procedure.
Assists patients with activities of daily living by performing duties such as providing personal care including assisting with toilet needs, providing comfort measures such as positioning and assisting with nourishments including serving and collecting meal trays and feeding patients.

Graduation from a recognized program for Practical Nurses, or an equivalent combination of education, training, and experience.
Current full practicing licensure with the British Columbia College of Nurses & Midwives (BCCNM).

We provide assessment and treatment services to improve our clients’ mental health and reduce their risk of re-offending. Our Brockville site offers 2 primary programs within the Integrated Forensic Program – a Secure Treatment Unit (STU) and a Forensic Treatment Unit (FTU)
The Secure Treatment Unit (STU):
The St. Lawrence Valley Correctional and Treatment Centre-Secure Treatment Unit is a 100-bed hybrid correctional facility and mental health centre located on the same campus of the Brockville Mental Health Centre. This facility serves 25 other provincial correctional facilities from across Ontario, and provides comprehensive mental health and criminogenic needs assessments, treatment and discharge planning services for seriously mentally ill adult male offenders serving a provincial sentence (less than two years).

Job Summary: As a member of an interdisciplinary team, you will be expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RPN will be responsible for working with individuals from diverse backgrounds, contributing to Enhanced Specialized Assisted Living Program development, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. What You Will Do:
Oversee the assessment of clients to determine suitability for program participation.
Identifying changes in appearance/behaviors, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviors.
Participating in service planning and development of personal goals for clients to transition to their permanent discharge location.
Providing support with behaviors arising from cognitive issues, intervening in conflicts between residents, and contacting appropriate emergency services in crisis situations.
Maintaining all records of service and ensuring supplies and equipment are readily available.
Liaising with physicians, pharmacists and other health care providers regarding client care.
Advising, coaching and supporting staff regarding medical/healthcare questions and concerns.
What You Bring: Must Haves
Current registration with the College of Nurses of Ontario and able to practice without restrictions.
Medication Administration Certification or completion of an RPN education preparation program that includes medication administration.
Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioral challenges who have been longer term hospital patients.
Knowledge and proven experience working with clients in a PSR (Psychosocial Rehabilitation) approach.
Knowledge of social services and community services for adults and seniors.
Knowledge of and experience in delivering direct services in accordance with psychosocial ‘recovery’ principles.
Ability to organize and work with individuals and groups in the areas of assessment, problem solving and crisis management; Ability to interact with clients in a positive and supportive manner.
Physically able to perform demanding body mechanics (i.e. lifting, transferring, Hoyer, etc.).
First Aid and CPR certification.
Good oral and written skills.
Excellent computer skills.
Ability to work effectively in a team environment

Ability to communicate in a different language, an asset.
Understanding of cultural competency and anti-oppression framework.
