Reporting to the WRHA Regional Lead, Health Services, Continuing Care & Community Health Services, and a member of WRHA’s senior leadership clinical operations’ team, the Executive Director is responsible for building and managing a multidisciplinary team regionally and oversight for the continuing care portfolio, that includes 39 long term care facilities, 10 supportive housing sites, 2 chronic care sites, healthy aging & seniors care. Leading the development and implementation of strategic and operational service planning (e.g. service operations, financial/ performance objectives and human resource priorities) for Continuing Care, the Executive Director is also responsible for ensuring service delivery, partnering closely with clinical supports and operational leadership across multiple service partners to ensure alignment with the WRHA’s strategic and operating plan and support patients/ residents/ clients in accessing health services. The portfolio has a wide range of internal and external partnerships and managing relationships in accordance with service purchase agreements.
The Executive Director, Continuing Care, is a key senior leader, responsible for operations management across multiple sites and participates in operational clinical services planning, implementation and monitoring/ evaluation of services within the portfolio.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
