Responsibilities of the position include, but may not be limited to:
Deliver primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO) and Bill 179, including comprehensive health assessments for those who use substances;
Provision of primary healthcare grounded by the five principles of Primary Healthcare as outlined by the World Health Organization;
Monitor client health status and investigation results; identify barriers to health and facilitate access to resources; collect and document statistical data;
Deliver client education through individual and group health education sessions;
Provide nursing care at various drop-in locations and participate in outreach activities;
Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs; collaborate with colleagues to identify population trends, develop research questions and/or conduct research studies;
Advocate for individual client needs to ensure access to effective care, follow-up and resources from individual and systemic levels; support and provide access for clients to low-barrier services at Street Health;
Participate in the evaluation of NP practice and the ongoing development and improvement of the NP role within Street Health; collaborate and consult with multidisciplinary staff within Street Health and partnering organizations to facilitate comprehensive primary care provision;
Initiate and/or participate in systemic advocacy activities directed at achieving health equity by improving health service delivery, eliminating stigma and developing healthy public policy;
Act as a preceptor to NP and Registered Nursing (RN) students and mentor new staff members;
Provide awareness & education to students, staff members, community health care institutions and community agencies regarding the communities we serve; facilitate knowledge translation by educating colleagues on topics related to client care;
Provide back-up services to Overdose Prevention Site (OPS) when needed;
Contribute to program and policy development; promote the mission, vision and values of Street Health; and
All other duties as may be assigned within the scope of this position.
Qualifications & Requirements :
Master’s Degree in Nursing.
Current registration as a Registered Nurse – Extended Class (Nurse Practitioner), in good standing with the College of Nurses of Ontario.
Minimum five years’ nursing experience including demon strated knowledge and commitment to the principles of harm reduction.
Demonstrated knowledge of mental health disorders, substance use treatment & harm reduction modalities and social determinants of health.
Relevant experience working within a community-based, not-for-profit organization focused on nursing outreach, harm reduction and mental health supports.
Experience advocating for clients, individually and at a systemic level.
Strong clinical assessment and documentation skills.
Strong oral and written communication skills; strong computer skills and experience using EMR software.
Experience working with marginalized groups and strong understanding of systems and issues affecting individuals who experience mental health disorders and homelessness is an asset.
Experience working within a team/organization harm reduction approach is an asset.
Certification in Advanced Nursing Footcare, or interest and willingness to attend training.
Ability to carry out duties independently while maintaining accountability to agency.

As a Mental Health and Addictions Outreach Nurse with VCH, you will:
Provide triage and outreach services that are inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions.
Conduct mental and physical status assessments and assesses clients’ physiological, psychological, sociocultural and spiritual needs to determine priority of care requirements based on client needs, availability of resources and best practices.
Provide counseling and assists client with reducing repeated contact with and use of acute and community healthcare services/programs/resources.
Assess high risk situations and provides crisis intervention.
Collaborate with interdisciplinary team to develop, implement and adjust client’s care plan and meet the client’s need for integrated services and continuity of care across the continuum.
Facilitate the movement of clients between acute care, community agency support site and designated health centre.
Participate in case conferences/meetings with members of the multidisciplinary team.
Assist clients with attendance to needed services such as medical appointments.
Qualifications
Education and Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Diploma in Nursing or Psychiatric Nursing plus two (2) years’ recent related experience working with individuals with mental health and addictions illness or an equivalent combination of education, training and experience.
Valid BC Drivers License. Local area travel may require the use of a personal vehicle.
Knowledge and Abilities
Knowledge of the principles and practices of a client and family centered recovery model in mental illness.
Broad knowledge of crisis intervention and counseling skills.
Broad knowledge of mental health illness and treatment.
Broad knowledge of psychopharmacology (indication and side effects).

Broad knowledge of other facilities and community resources.

The interdisciplinary team will work closely with community service providers including shelter and supportive housing staff to provide a community-based, healthcare-led crisis response, post-crisis follow up, and service navigation. Bloom Clinic is a program of WellFort Community Health Services and is committed to providing treatment, care, and support to individuals living with, affected by, or at risk of Hepatitis C (HCV) and/or HIV across the Region of Peel. Bloom Clinic is committed to providing community-based testing, treatment, and support to people who are experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, and Indigenous community members.
The NP will integrate his/her knowledge of advanced nursing practice and theory, health assessment and diagnosis, therapeutic management, health promotion, disease prevention, and chronic disease management to provide primary health care to individuals, families, groups, and communities. The NP will work both independently and collaboratively with his/her clients, interdisciplinary team, and other healthcare providers in delivering high-quality clinical care and outreach activities.
While the proportion of time allocated to different functions may vary according to service needs, the advanced nursing role encompasses a combination of direct care, health promotion, outreach, education, advocacy, community development, program development, and evaluation.

As a Indigenous Mental Health & Substance Use Outreach Nurse with Vancouver Coastal Health you will:
Work as part of a multidisciplinary team to provide crisis intervention, triage and outreach services that are inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions.
Conduct mental and physical status assessments and assesses clients’ physiological, biopsychosocial, and cultural support needs to determine priority of care requirements based on client needs, availability of resources and best practices.
Provide counseling and assists client with reducing repeated contact with and use of acute and community healthcare services/programs/resources. Assesses high risk situations and provides crisis intervention. Collaborates with interdisciplinary team to develop, implement and adjust client’s care plan and meet the client’s need for integrated services and continuity of care across the continuum. Facilitates the movement of clients between acute care, community agency support site and designated health centre.
Participate in case conferences/meetings with members of the multidisciplinary team.
Assist clients with attendance to needed services such as medical appointments.
The position also works in tandem to Indigenous Health Outreach, in addition to other VCH Vancouver Community services, and liaises and works with the local First Nation community, other Indigenous serving organizations, community resources, and healthcare providers to assist Indigenous clients to access support services such as benefits or assistance as appropriate.
Provide care within the interdisciplinary model through trauma and violence-informed practice, harm reduction, cultural safety, and a recovery-oriented care approach.
Use a relational and strengths-based perspective to support the client to identify care goals.
Practice in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards (professional & practice) and code of ethics for registered nurses or registered psychiatric nurses. Practices within the scope of practice as set by the organization, as per VCH policies and guidelines/clinical practice documents and within own competence.
This practice is aligned with the vision, mission, goals and values of the organization as well as with the concept of the resident/client and family centred care model.
Qualifications
Education & Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Diploma in Nursing or Psychiatric Nursing
Two (2) years’ recent related experience working in Indigenous-led or Indigenous-serving organizations providing mental health and substance use clinical services in community-based settings or an equivalent combination of education, training and experience. Valid BC Drivers License.
Local area travel may require the use of a personal vehicle.
Pursuant to Section 41 of the British Columbia Human Rights Code, preference will be given to individuals of Indigenous identity (First Nations, Inuit, M�tis).
Knowledge and Abilities
Comprehensive knowledge and demonstrated ability to provide culturally safe care, has completed Indigenous Cultural Safety training.
Demonstrated understanding and in-depth knowledge of First Nations, Inuit, and M�tis Peoples and history.
Demonstrated understanding of provincial and federal guiding principles and documents, for example In Plain Sight Report, and Calls to Action, to eliminate systemic racism in the health care sector.
Demonstrated ability to identify and recognize current health care system gaps and barriers for Indigenous individuals and communities seeking health care.
Demonstrated ability to identify and recognize power inequities within the healthcare system associated with ongoing impacts of colonialism.
Demonstrated knowledge of the principles of recovery in mental health and substance use.
Demonstrated knowledge and experience in Trauma and Violence Informed Practice
Demonstrated knowledge and ability to apply a Harm Reduction approach to care.
Knowledge of the principles and practices of a client and family centered recovery model in mental illness.
Knowledge of the Mental Health Act as it applies to the rights and obligations of clients and staff.
