Position Summary:
Reporting to the Patient Care Manager, Toronto Grace Health Centre is currently seeking a Nurse Practitioner at our Scarborough location. The Nurse Practitioner is an active member of the interdisciplinary team that also includes a social worker, occupational therapist, and physiotherapist. The NP has clinical and leadership expertise in the provision of quality care for diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting. They will act as a leader and mentor for the professional nurses, allied health professionals, administrative and other supportive staff on the unit.
Responsibilities:
Provide client and family-centered care independently and in collaboration with other team members.
Demonstrate all the Advanced Nursing Practice Competencies outlined by the Canadian Nurses Association including clinical practice, research, leadership, consultation, and collaboration.
Conduct the patient care health assessment and diagnosis, therapeutic management, and health promotion and prevention of illness and injury.
Work in collaboration with the interdisciplinary team in a shared-care model of practice providing cross coverage.
Liaise regularly with external health organizations.
Support best practice in seniors’ care and other health care providers where appropriate.
Consult on patient/client care, providing advice, expertise, and consultation services.
Qualifications:
Currently registered with the CNO as a Registered Nurse in the Extended Class.
Currently have RNAO or NPAO membership with LAP or equivalent.
Additional post graduate education or certification in Mental Health and Addictions services / management related areas is a strong asset.
Minimum Three (3) years of relevant clinical experience, including experience working with clients with complex mental health illnesses.
Experience working with persons with Dementia or cognitive impairment.
Successfully completed a Primary Health Care Nurse Practitioner Program in Ontario or equivalent and hold a master’s degree in nursing with a Primary Health Care Nurse Practitioner Program (family all ages).
Your clinical focus will be with chronic disease management, geriatric nursing care, mental health, and addictions services where there is an emphasis on collaborative practice within an inter-professional milieu.
Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, push and pulling.
You have demonstrated clinical leadership skills and a commitment to continued participation in research.
Must have received at least two doses of a COVID-19 vaccine approved by Health Canada prior to start date and submit evidence of such. Street Health is a community-based agency providing innovative physical and mental health programs and advocacy to homeless and insecurely housed individuals in downtown Toronto for over 30 years. Street Health, a not-for-profit organization located within the downtown-east neighborhood of Toronto, works to improve the health and wellbeing of people who are homeless and under-housed by addressing the social determinants of health. Our organization provides services in the areas of innovative nursing, mental health, harm reduction, Overdose prevention, Safer Supply, outreach, identification replacement programs, and through advocacy, research, and education.
Responsibilities of the position include, but may not be limited to:
Deliver primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO) and Bill 179, including comprehensive health assessments for those who use substances;
Provision of primary healthcare grounded by the five principles of Primary Healthcare as outlined by the World Health Organization;
Monitor client health status and investigation results; identify barriers to health and facilitate access to resources; collect and document statistical data;
Deliver client education through individual and group health education sessions;
Provide nursing care at various drop-in locations and participate in outreach activities;
Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs; collaborate with colleagues to identify population trends, develop research questions and/or conduct research studies;
Advocate for individual client needs to ensure access to effective care, follow-up and resources from individual and systemic levels; support and provide access for clients to low-barrier services at Street Health;
Participate in the evaluation of NP practice and the ongoing development and improvement of the NP role within Street Health; collaborate and consult with multidisciplinary staff within Street Health and partnering organizations to facilitate comprehensive primary care provision;
Initiate and/or participate in systemic advocacy activities directed at achieving health equity by improving health service delivery, eliminating stigma and developing healthy public policy;
Act as a preceptor to NP and Registered Nursing (RN) students and mentor new staff members;
Provide awareness & education to students, staff members, community health care institutions and community agencies regarding the communities we serve; facilitate knowledge translation by educating colleagues on topics related to client care;
Provide back-up services to Overdose Prevention Site (OPS) when needed;
Contribute to program and policy development; promote the mission, vision and values of Street Health; and

Qualifications & Requirements :
Master’s Degree in Nursing.
Current registration as a Registered Nurse – Extended Class (Nurse Practitioner), in good standing with the College of Nurses of Ontario.
Minimum five years’ nursing experience including demon strated knowledge and commitment to the principles of harm reduction.
Demonstrated knowledge of mental health disorders, substance use treatment & harm reduction modalities and social determinants of health.
Relevant experience working within a community-based, not-for-profit organization focused on nursing outreach, harm reduction and mental health supports.
Experience advocating for clients, individually and at a systemic level.
Strong clinical assessment and documentation skills.
Strong oral and written communication skills; strong computer skills and experience using EMR software.

Responsibilities of the position include, but may not be limited to:
Deliver primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO) and Bill 179, including comprehensive health assessments for those who use substances;
Provision of primary healthcare grounded by the five principles of Primary Healthcare as outlined by the World Health Organization;
Monitor client health status and investigation results; identify barriers to health and facilitate access to resources; collect and document statistical data;
Deliver client education through individual and group health education sessions;
Provide nursing care at various drop-in locations and participate in outreach activities;
Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs; collaborate with colleagues to identify population trends, develop research questions and/or conduct research studies;
Advocate for individual client needs to ensure access to effective care, follow-up and resources from individual and systemic levels; support and provide access for clients to low-barrier services at Street Health;
Participate in the evaluation of NP practice and the ongoing development and improvement of the NP role within Street Health; collaborate and consult with multidisciplinary staff within Street Health and partnering organizations to facilitate comprehensive primary care provision;
Initiate and/or participate in systemic advocacy activities directed at achieving health equity by improving health service delivery, eliminating stigma and developing healthy public policy;
Act as a preceptor to NP and Registered Nursing (RN) students and mentor new staff members;
Provide awareness & education to students, staff members, community health care institutions and community agencies regarding the communities we serve; facilitate knowledge translation by educating colleagues on topics related to client care;
Provide back-up services to Overdose Prevention Site (OPS) when needed;
Contribute to program and policy development; promote the mission, vision and values of Street Health; and

Qualifications & Requirements :
Master’s Degree in Nursing.
Current registration as a Registered Nurse – Extended Class (Nurse Practitioner), in good standing with the College of Nurses of Ontario.
Minimum five years’ nursing experience including demon strated knowledge and commitment to the principles of harm reduction.
Demonstrated knowledge of mental health disorders, substance use treatment & harm reduction modalities and social determinants of health.

Join our growing pharmacy team at Hamilton Health Sciences in providing safe and effective medication while The Nurse Practitioner (NP) for the General Pediatrics – Complex Care Service at McMaster Children’s Hospital is dually accountable to the Director of the Child and Youth Acute Care Program and the Chief of Interprofessional Practice. The NP will provide leadership and promote excellence in pediatric family centered nursing care through expert clinical practice, education, consultation and research. The NP supports the clinical activity of the General Pediatrician – Complex Care and the associated patient care and interventions through the Ambulatory service including off site affiliated satellite clinics in LHIN 3 & 4. This would include the areas of the pediatric emergency department, inpatients and the outpatient clinics.
Collaborating with the interdisciplinary team members, and using a patient and family centered model of care, the NP is responsible for providing direct patient care to their full scope of practice, coordination of care, discharge planning, and the coordination of related community and other resources for this patient population; mentor nursing and other health professional students as appropriate, and participate in continuing education activities to integrate the role of the NP in the program and academic settings. The NP will also initiate, participate, and/or lead programs, presentations, publications, evaluations, research initiatives and other system changes to continue the enhancement of Interprofessional care teams. A capacity for critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
Experience working with confidential and time sensitive documents and information.
Excellent interpersonal skills and the ability to multi task in a fast paced environment.
Experience working in a value based non-profit or social services organization.
Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
Demonstrated proficiency in use of computers and various computer software applications, such as Electronic Medical Record (EMR).
Ability to speak languages relevant to the residents of the organization’s community is an asset.

VACCINATION REQUIREMENT:
As a condition of employment, new WellFort staff must be fully vaccinated unless they have received an exemption from vaccination under the Human Rights Code. Proof of COVID-19 vaccination status will be required before the first day of work or, proof of religious or medical exemption, if or where applicable.
Fully vaccinated is defined as having received the completed series of an accepted COVID-19 vaccine, as recommended by the Office of the Chief Medical Officer of Health and having received the final dose at least 14 days before your employment start date.
The candidate will be asked to provide WellFort with proof of full vaccination, prior to their employment start date. Acceptable proof is a Ministry of Health Dose Administration Receipt (or such other proof of vaccination that the Province of Ontario sanctions). This can be obtained through the Provincial portal https://covid-19.ontario.ca/get-proof/.
