SALUS House serves a sub-acute patient population, with an expected stay of four weeks. Services include short-term assessment, crisis counseling, medication adjustments, and stabilization.

Qualifications
Education & Experience
Current practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Three (3) years' recent, related nursing experience in an acute psychiatric environment, including one (1) years’ experience in client education and/or group facilitation, or an equivalent combination of education, training and experience.
Knowledge, Skills & Abilities
Broad knowledge of nursing science theory, and practice within a patient centred care model.
Comprehensive knowledge of the BCCNM standards of practice for Registered Nurses or Registered Psychiatric Nurses.

Broad knowledge of community mental health services, resources, and referral procedures

Registered Nurse (RN) - 2 Full-Time, Temporary (20 months) - HART Hub - Barrie, ON
The Canadian Mental Health Association, Simcoe County Branch seeks two (2) RNs, who, in accordance with the agency’s policies, standards, and guidelines, provides client-centered primary care health care through the Homelessness, Addictions, Recovery, Treatment (HART) Hub in Barrie. This position will support the CMHA NP, Physicians within the HART Hub and HART Hub program to integrate outreach-informed primary care with other health and social services delivered by CMHA SCB and community partners.
DUTIES AND RESPONSIBILITIES
· Help organize and deliver primary care in both a clinic and outreach environment.
The HART Hub is a new program that includes mobile supports in the community. Up to 75% of services could be provided outside the NP clinic.
· Perform all aspects of nursing care in accordance with standards of nursing practices established by the College of Nurses of Ontario.
· Participate with an NPs in a multi-disciplinary consultive, client-centered recovery orientated specialized mental health treatment/medical service delivery model.
· Provide the majority of the treatment, rehabilitation and support services, in addition to clinical & psychosocial rehabilitation and recovery for persons who have the most serious mental illness, have severe symptoms and functional impairments, and have not benefited from traditional outpatient programs.
· Complete physical health assessments to thoroughly determine the health status of clients as well as any medical conditions present, and ensure appropriate treatments, follow-up, and support are provided.
· Provide members of the HART Hub team, clients, families, and others with specialized consultation and education related to nursing issues.
· Responsible for assessing and documenting clients’ mental illness and addictions symptoms and behaviors and response to medication; and monitor for medication side effects.
· Responsible for delivery, observation, monitoring of compliance, and documentation of medication.
· Develop, implement, and document, transition plans in conjunction with clients and other HART Hub team members.
· Participate in clients’ individualized recovery plans, including but not limited to addictions, psychiatric illness or symptom reduction, housing activities of daily living, daily structured employment and family and social relationships.
· Provide counselling to assist clients in accomplishing their goals and managing their symptoms/day-to-day stressors.
· Access, coordinate, and advocate for services/resources to meet client needs.
· Consult with other service providers to promote continuity of care for the clients.
· Work with HART Hub Team members to provide support, monitoring and advocacy for clients to obtain and maintain housing. Participate in HART Hub team meetings as needed to ensure continuity of care and wrap around supports.
· Responsible for transporting clients in the community and, on occasion, outside of the geographical area.
REQUIRED QUALIFICATIONS:
Minimum five (5) years experience in primary care/mental health and addictions nursing (community-based preferred).
Demonstrated administrative /coordination experience.
Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
Fluency in French or another second language would be an asset.
Legally entitled to work in Canada.
A Registered Nurse with membership in good standing with the College of Nurses of Ontario.
SALARY SCALE:
RN: $71,544 - $78,042 *per annum
HOURS OF WORK:
Thirty-five (35) hours per week, Monday to Friday

The Canadian Mental Health Association, Simcoe County Branch seeks two (2) RNs, who, in accordance with the agency’s policies, standards, and guidelines, provides client-centered primary care health care through the Homelessness, Addictions, Recovery, Treatment (HART) Hub in Barrie. This position will support the CMHA NP, Physicians within the HART Hub and HART Hub program to integrate outreach-informed primary care with other health and social services delivered by CMHA SCB and community partners.
DUTIES AND RESPONSIBILITIES
· Help organize and deliver primary care in both a clinic and outreach environment.
The HART Hub is a new program that includes mobile supports in the community. Up to 75% of services could be provided outside the NP clinic.
· Perform all aspects of nursing care in accordance with standards of nursing practices established by the College of Nurses of Ontario.
· Participate with an NPs in a multi-disciplinary consultive, client-centered recovery orientated specialized mental health treatment/medical service delivery model.
· Provide the majority of the treatment, rehabilitation and support services, in addition to clinical & psychosocial rehabilitation and recovery for persons who have the most serious mental illness, have severe symptoms and functional impairments, and have not benefited from traditional outpatient programs.
· Complete physical health assessments to thoroughly determine the health status of clients as well as any medical conditions present, and ensure appropriate treatments, follow-up, and support are provided.
· Provide members of the HART Hub team, clients, families, and others with specialized consultation and education related to nursing issues.
· Responsible for assessing and documenting clients’ mental illness and addictions symptoms and behaviors and response to medication; and monitor for medication side effects.
· Responsible for delivery, observation, monitoring of compliance, and documentation of medication.
· Develop, implement, and document, transition plans in conjunction with clients and other HART Hub team members.
· Participate in clients’ individualized recovery plans, including but not limited to addictions, psychiatric illness or symptom reduction, housing activities of daily living, daily structured employment and family and social relationships.
· Provide counselling to assist clients in accomplishing their goals and managing their symptoms/day-to-day stressors.
· Access, coordinate, and advocate for services/resources to meet client needs.
· Consult with other service providers to promote continuity of care for the clients.
· Work with HART Hub Team members to provide support, monitoring and advocacy for clients to obtain and maintain housing. Participate in HART Hub team meetings as needed to ensure continuity of care and wrap around supports.
· Responsible for transporting clients in the community and, on occasion, outside of the geographical area.
REQUIRED QUALIFICATIONS:
Minimum five (5) years experience in primary care/mental health and addictions nursing (community-based preferred).
Demonstrated administrative /coordination experience.
Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
Fluency in French or another second language would be an asset.
Legally entitled to work in Canada.
A Registered Nurse with membership in good standing with the College of Nurses of Ontario.
SALARY SCALE:
RN: $71,544 - $78,042 *per annum
HOURS OF WORK:
Thirty-five (35) hours per week, Monday to Friday
