Bella Coola General Hospital has 10 acute bed, 5 long term care beds and a 3 bed emergency department open 24/7, along with a laboratory, diagnostic imaging services, Telehealth, Mental Health, Public Health, Home & Community Support Services. Also within the hospital are a medical clinic and a pharmacy. As a MHSU Case Coordinator Outreach Nurse (RN/RPN) with VCH you will:  Work as part of a multidisciplinary team, performing intake, triage, and outreach functions and services to individuals with serious mental health and/or substance use issues. Conduct mental health and physical assessments, consult, confer and collaborate with the interdisciplinary team and family members to develop, implement, evaluate and modify individualized care plans to meet clients need for integrated services and continuity of care across the continuum. Establish, maintain and enhance therapeutic relationships, provide counselling support, administer prescribed medicines, monitor client response. Prepare and maintain client records according to established standards, participate in team meeting, engage in team planning and quality improvement activities and maintains and updates own knowledge. Qualifications Education & Experience Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Diploma in Nursing or Psychiatric Nursing plus three (3) years’ recent related experience working with individuals with mental health and substance use issues or an equivalent combination of education, training and experience. Valid BC Driver’s license and local area travel may require the use of a vehicle, as required.
