Registered Social Worker, Clinical Program

 Join our interdisciplinary team as we work collaboratively to deliver clinical and mental health support to patients of all ages living with type 1 diabetes.

Part-time Position (22.5-30 hours per week)

Hourly Rate Range: $37.50 – $44.40

Schedule: Flexible weekdays and shift lengths

Location: Onsite with the flexibility to work remotely

KEY RESPONSIBILITIES

• Formulate and deliver assessment and evidence-based treatment plans in partnership with the patient, family and interdisciplinary team to achieve progress toward goals (including management of crisis and life-threatening behaviours).
• Conduct individual, couple, and family therapy sessions across all ages of the lifespan with a special focus on the pediatric population.
• Develop and facilitate group psychoeducation and therapeutic sessions.
• Ensure quality and integrity of all patient information through use of electronic medical records.
• Work collaboratively with the interdisciplinary team to ensure continuity and efficiency of the education and support being provided to our patients and families.

QUALIFICATIONS

• Master of Social Work degree 
• Clinical and counselling experience in the assessment and treatment of children, youth and complex families in a hospital-based or community-based clinical team
• Current membership in good standing with the OCSWSSW
• Proven ability to practice with clinical autonomy with a commitment to collaborative practice
• Proof of professional liability insurance
• Experience with evidence-based treatment modalities, including Cognitive Behavioural Therapy, Dialectical Behavioural Therapy, motivational interviewing, mindfulness, crisis intervention, family therapy and group facilitation
• Accurate, thorough and timely documentation skills
• Demonstrated commitment to ongoing and self-directed professional education and development
