Job Description

We are currently looking for someone to join our leadership team in the role of Director, Professional Practice, Research and Education.

Reporting to the Chief Innovation and Operations Officer, the Director of Professional Practice, Research and Education works closely with all portfolios to advance improvement in quality care delivery in alignment with an integrated care delivery model and collaborative care practice approach.  The Director oversees a team of clinical practice specialists and Professional Practice Advisors with accountability for the development and implementation of evidence-based clinical practices, procedures and strategies to advance best practice and integrated care delivery across CarePartners’ nursing, rehabilitation and personal support teams at a local level.   The Director provides enterprise wide leadership in program and curriculum development for quality practice education; supports the development of, and champions, CarePartners’ digital and integrated care strategies; develops structures and processes for the advancement of integrated care and education; ensures compliance with quality and risk management initiatives including patient safety; and makes certain all activities align to CarePartners’ strategic priorities, policies and procedures. 

PRIMARY DUTIES AND RESPONSIBILITIES

Advances Integrated Care and Professional Practice

  • Provides strategic leadership and advice around professional practice issues pertaining to all programs and services delivered by CarePartners with accountability in the domains of: practice standards, quality practice education, and research  
  • Champions a culture that is committed to, and passionate about, patient-centred, high quality care and the identification of transformative opportunities in enabling point-of-care excellence in every patient and caregiver interaction  
  • In collaboration with Patient Care and Operations leadership and Organizational Effectiveness, Quality and Risk portfolio, ensures strategies are implemented that will advance excellence in clinical care outcomes from a population health lens 
  • Accountable for the design, implementation, evaluation and maintenance of core quality practice standards (for regulated and unregulated team members) and clinical programs necessary to advance interprofessional and professional care in alignment with leading practices, college standards, and principles of collaborative care and integrated care models
  • Contributes to the establishment of CarePartners’ strategic priorities relevant to professional practice and ensures execution of an evidence informed annual planning cycle within the department that is informed by innovations and emerging trends in technology, health care, and adult education 
  • Provides strategic direction, advice anchored in evidence to inform enterprise wide efficiencies and effectiveness in the delivery of professional practice infrastructure across the local sites 
  • Ensures mitigation strategies are in place to address identified risk and safety issues related to the organization’s enterprise risk map and supports with relevant departments key quality initiatives that drive patient and staff safety 
  • Accountable for the establishment of a patient and caregiver engagement framework and associated processes that will enable consistent and ongoing patient, family and caregiver participation in CarePartners’ key organizational activities 
  • Leads or participates in internal project teams and committees, as assigned 

Education 

  • Provides oversight and direction to the design, evaluation, and orientation, mentoring and coaching for Patient Care and Operations frontline team members and leadership 
  • Ensures clinical programming and education is evidence-informed, relevant to changing models of care delivery and enables high quality patient care delivery within a dynamic home and community care context 
  • Champions the use of digital learning platforms and identifies infrastructure necessary to enable nimble and responsive learning for frontline team members
  • Supports local site-specific training and coaching initiatives that are informed by key performance indicators; such as, patient experience and outcomes measures, including ETMS and CCEE data 

Research

  • Accountable for the establishment of a research framework that aligns with CarePartners’ strategic priorities 
  • Ensures innovative practices and research outcomes and evidence are inform the establishment of core processes related to patient care at local care delivery sites 
  • Participates in research and knowledge mobilization opportunities, with academic institutions and provincial organizations through conferences and research projects, presenting on behalf of CarePartners Other duties as assigned

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Regulated Health Care Professional in good standing with their respective regulatory college
  • Masters preparation in a health-related discipline required 
  • Minimum 3 years’ experience in leading clinical practice initiatives in a home and community care environment
  • In-depth knowledge regarding operations, structures and functions the home and community care sector 
  • Minimum 5-7 years clinical experience with progressive leadership 
  • Demonstrated experience in leading a high performing team 
  • Experience in applying project management principles
  • Demonstrated leadership in successfully executing on large scale, complex initiatives
  • Knowledge of adult learning principles 
  • Experience in implementing organization wide training and educational programming an asset
  • Knowledge and experience in best practices for disciplines and quality measurement
  • Strong proficiency in all aspects of Microsoft office, with proven skills in Word and Excel;
  • Keen attention to detail, ability to prioritize, multitasking skills and organizational skills;
  • Strong written communication, proofreading and editing skills required to draft and/or generate correspondence/documents;
  • Ability to multi-task with an attention to detail, demonstrating organizational, prioritization, and time-management skills, as well as proven ability to meet deadlines
  • Strong analytical skills;
  • Demonstrated ability to handle highly sensitive and confidential information.
  • Ability to travel within Ontario on an occasional basis
