Position Summary:
Reporting to the Patient Care Manager, Toronto Grace Health Centre is currently seeking a Nurse Practitioner at our Scarborough location. The Nurse Practitioner is an active member of the interdisciplinary team that also includes a social worker, occupational therapist, and physiotherapist. The NP has clinical and leadership expertise in the provision of quality care for diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting. They will act as a leader and mentor for the professional nurses, allied health professionals, administrative and other supportive staff on the unit.
Responsibilities:
Provide client and family-centered care independently and in collaboration with other team members.
Demonstrate all the Advanced Nursing Practice Competencies outlined by the Canadian Nurses Association including clinical practice, research, leadership, consultation, and collaboration.
Conduct the patient care health assessment and diagnosis, therapeutic management, and health promotion and prevention of illness and injury.
Work in collaboration with the interdisciplinary team in a shared-care model of practice providing cross coverage.
Liaise regularly with external health organizations.
Support best practice in seniors’ care and other health care providers where appropriate.
Consult on patient/client care, providing advice, expertise, and consultation services.
Qualifications:
Currently registered with the CNO as a Registered Nurse in the Extended Class.
Currently have RNAO or NPAO membership with LAP or equivalent.
Additional post graduate education or certification in Mental Health and Addictions services / management related areas is a strong asset.
Minimum Three (3) years of relevant clinical experience, including experience working with clients with complex mental health illnesses.
Experience working with persons with Dementia or cognitive impairment.
Successfully completed a Primary Health Care Nurse Practitioner Program in Ontario or equivalent and hold a master’s degree in nursing with a Primary Health Care Nurse Practitioner Program (family all ages).
Your clinical focus will be with chronic disease management, geriatric nursing care, mental health, and addictions services where there is an emphasis on collaborative practice within an inter-professional milieu.
Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, push and pulling.
You have demonstrated clinical leadership skills and a commitment to continued participation in research.
Must have received at least two doses of a COVID-19 vaccine approved by Health Canada prior to start date and submit evidence of such. Must maintain Covid-19 Vaccination schedules as approved by Health Canada. What you’ll do
Maintain patient records by collecting and documenting information including health care information regarding patients and their families, nursing observations, telephone communications, interventions, teaching, and care evaluations to ensure that patient records are current and complete.
Plan and provide holistic, anti-racist direct care rooted in cultural humility for the infants, children, and youth by performing activities such as reviewing patient/ family history, developing a care plan specific for the patient and family, and meeting with other members of the health care team to discuss and review the care plan. Collaborate with other members of the health care team to evaluate patient and family centered care in the practice setting.
Provide education to patients and their support systems, in a manner acceptable to those individuals that facilitates the successful delivery of perianesthesia patient care by providing, explaining, and demonstrating care procedures and practices.
Identify own learning needs, reviews these with the Program Manager or delegate, and engages in a variety of activities to meet these needs.
Participate in the orientation and continuing education of all members of the care team by preceptoring and role-modelling.
Act as a clinical resource/support person to patients, nursing and health and care team professionals by explaining the theoretical rationale for nursing decisions.
Provide input to the Program Manager, or delegate, regarding policies, procedures, and operational requirements by observing service needs and providing spoken or written suggestions whenever improvement opportunities are noticed, which address patient and family needs and promote effective use of resources and advocates for patient care.
Maintain a safe environment for patients, families and advocate for the patient and their support system in all aspects of the care interaction.

Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses and Midwives (BCCNM).
CPR Training – Basic Cardiac Life Support (BCLS) – Level 1.
One year of recent pediatric PACU experience in a tertiary level setting or one year of recent pediatric critical care experience in a tertiary level setting or a combination of Pediatric Critical Care Courses, Post Anesthetic nursing courses, PerioAnesthesia Nursing Course or an equivalent combination of education and experience.

What you’ll do
Perform role and specialty specific clinical workflow assessments and translates requirements to digital health/information technology leads. Lead in the development of organizational and/or departmental clinical change management policies and procedures as they relate to the electronic health record and clinical practice process redesign as it pertains to the automation of professional practice. Lead the identification and assists in the development of new and changing role and specialty specific functions and processes in the department/organization as they relate to the use of clinical information systems.
Lead the system design life cycle in the areas of design, content development, validation, implementation, stabilization and evaluation of electronic health information systems and their associated data dictionaries, databases, tables, reports and related standards, protocols and procedures to support clinical uptake of electronic health and information records. Participate in custom report development and data analysis for decision-making.

What you’ll do
Perform role and specialty specific clinical workflow assessments and translates requirements to digital health/information technology leads. Liaise with multiple clinical groups, project teams, and members of the organization and peer health authorities to assess technology gaps and clinical needs for providers and clinical users of the electronic health record system. Provide subject matter expertise in system design and evaluation of clinical content, troubleshooting and end–user support.
Liaise with the Digital Health department, organizational programs/departments and partner affiliates as the role and informatics specialty specific expert and resource on system wide practice and workflow change initiatives, practice innovation and redesign, documentation/nomenclature standards and the electronic health record.
Champion a collaborative team-based framework that incorporates resources from other departments, programs, and vendor partners as it relates to clinical systems implementations. Lead and participates in steering committees and taskforces for PHSA clinical informatics and information technology initiatives.
Lead in the development of organizational and/or departmental clinical change management policies and procedures as they relate to the electronic health record and clinical practice process redesign as it pertains to the automation of professional practice. Lead the identification and assists in the development of new and changing role and specialty specific functions and processes in the department/organization as they relate to the use of clinical information systems.
Lead the system design life cycle in the areas of design, content development, validation, implementation, stabilization and evaluation of electronic health information systems and their associated data dictionaries, databases, tables, reports and related standards, protocols and procedures to support clinical uptake of electronic health and information records. Participate in custom report development and data analysis for decision-making.
Provide leadership and/or coaching to nurses and other clinical staff based on knowledge and informatics experience on incorporating clinical informatics advances and electronic health records developments into practice.
Engage in the planning, coordination, scheduling, implementation and evaluation of upcoming releases including impact analysis, development of release notes and liaising with other subject matter experts to support development and delivery of education for end users to enhance adoption, including use of clinical decision support tools within the clinical workflows to support timely decision making.
Plan and lead adoption training sessions such as, workflow review sessions and one-on-one support sessions related to appropriate, safe and efficient use of clinical information systems/applications including applicable policies and related legislation regarding access to electronic health records.
