Health People. Healthy Communities. Serving the diverse communities of southeast Toronto and beyond, Sherbourne Health (Sherbourne) is dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and its diverse communities to achieve wellness. Sherbourne serves clients through its Family Health Team, which comprises nurses, mental health counsellors, physicians, outreach workers, dietitians, client resource workers, and health promoters. In addition to providing primary care, Sherbourne offers mental health services in both individual and group settings, outreach support chronic disease management services and respite care and support to people who are under-housed or homeless in its 14-bed in-patient program called ARC. Sherbourne is widely recognized for its efforts to reduce social, economic and other barriers to health care. Further, Sherbourne’s leading-edge health and support services for lesbian, gay, bisexual, trans, 2-Spirit and queer (LGBT2SQ) communities garner international attention.
Since its inception in 2002, Sherbourne’s dedicated staff, board of directors, volunteers and community partners have worked tirelessly to develop programs and services to meet the needs of the communities it serves. Sherbourne provides primary care and community program services through +70,000 client visits per year, of which newcomers to Canada, LGBTQ people and people who are homeless or under-housed are key client groups.
