A full-time position for a Director of Clinical Programs – Complex Continuing Care (CCC) is available at West Park Healthcare Centre, reporting to the Vice-President of Programs and CNE. The primary role of the successful candidate is the delivery of effective and innovative leadership to the CCC program. West Park is building a new hospital and transforming its 27-acre site into an integrated campus of care, enabling the hospital to evolve its rehabilitative programs to meet Ontario’s future healthcare needs.
In partnership with the Director Clinical Programs – Rehabilitation, Director of Interprofessional Practice and Clinical Education, the Clinical Program Director- CCC will promote seamless, efficient and effective patient care services. The Director will role model professionalism, fiscal accountability and a culture of safety for patients and staff. This role will appeal to a candidate interested in making significant contributions to a campus of integrated care, and partner with health care system partners to advance integrated and seamless healthcare services.
The successful candidate’s salary will be commensurate of qualifications and experience, and will be eligible for a benefits package which includes medical, dental, life insurance, long term disability insurance, group insurance and pension plan.
Success in the role calls for a nurse leader with a Master’s degree in Nursing, Health Administration or Business. You demonstrated ability to foster a culture whereby staff ‘enhance lives, inspire hope and encourage independence through caring relationships, leading practices, specialized services and partnership”. You are a strong leader with extensive experience leading in a complex and transformational environment. You will guide and support the strategic and operational planning including patient care delivery, access and flow, clinical effectiveness and efficiency, and human resource and financial planning. Leadership experience in acute medicine is considered an asset.
Candidates must hold licensure with the College of Nurses of Ontario.

West Park Healthcare Centre helps patients get their lives back by providing inpatient, outpatient and outreach services in specialized rehabilitative and complex care after a life-altering illness or injury such as lung disease, amputation, stroke, and traumatic musculoskeletal injuries. By 2021, West Park’s 27-acre site will be transformed into an integrated campus of care, including a new hospital building and additional community supports such as assisted living for persons with disabilities and seniors.
