Job Description

This newly created role will be accountable for developing and implementing system-level initiatives to advance integrated care and partnerships. This director will lead partnership projects and build system capacity by envisioning, developing and maintaining key relationships and partnerships that will better connect services across the system both within and outside Unity Health Toronto. 

Responsibilities & Activities:

  • Leads, champions and directs the development of transformation and partnership strategies for the organization.
  • Central point of contact and key organization networker for strategic stakeholder relations and partnerships.
  • Develops and implements an organizational transformation strategy that focuses on building a more integrated and inter-connected health system and advancing partnerships across the continuum of care.
  • Build and maintain relationships with a range of key primary care networks, community providers, hospitals and cross-sector agencies to advance a partnership strategy that is integrated, outcome-focused and transformative.
  • Ensures that the transformation and partnership strategy will align with and achieve the strategic priorities set out in the organization’s overall Strategic Plan and is tied to its Mission, Vision and Values.
  • Works with Senior Management and other key leaders managing the relationships of the partners.
  • Demonstrates advanced knowledge of the LEADS model of Leading Self, Leading Others and Leading Systems and personally models effective leadership behaviours in all aspects of their work.  


  • Consults with the senior leadership team around decisions that will have a substantial impact on system transformation.
  • Works with the VP, People and Transformation on appropriate direction to take to ensure the focus of partnership is directed within the goals of the organization.
  • Understands functional or site specific requirements and is able to deliver partnerships that establish what’s required to meet specific needs. 


  • At least 7 years of progressive experience system level leadership experience
  • Experience building relationships and partnerships related to health system integration
  • Strong and innovative health care leader with a track record in provincial health system integration and successful track record of executing on a range of initiative and inspiring system level change
  • Exceptional relationship developer, capable of inspiring teams and working with organizational partners to facilitate change while fostering a culture of creativity and innovation
  • Strong existing relationships across both the healthcare industry and the provincial government
  • Strong project management experience, including demonstrated ability to efficiently plan, organize, and manage complex, multiple projects simultaneously with limited supervision or direction
  • Superior critical thinking, analytical and problem-solving skills
  • Exceptional interpersonal, team-building and communication skills
  • Ability to build consensus and foster change
  • Outstanding integrity, initiative, creativity, and passion
