THE OPPORTUNITY

The Moir Centre for Complex Continuing Care opened in November 2016. This new unit aspires to be at the forefront of emerging best practices in CCC using a person centred approach to care. The Moir is a 39-bed unit that inhabits a bright and inviting space that is designed with patients and families in mind. Well staffed with an RN/ allied Health team of 90 as well as two Nurse Practitioners, an educator and a charge nurse to help support the manager. This team is dedicated to providing best in class care to their patients and support the families with loved ones in the Centre. We are looking for an enthusiastic and skilled leader who has the ability to continue the work done over the last year and a half to bring this vision to life and to propel the unit forward.

Reporting to the Program Director, Primary Care, Rehabilitation, Complex Continuing Care, and Seniors’ Services, and with accountability to the Program Chief and Medical Director of the Program, the Patient Care Manager, Complex Continuing Care is an integral member of the Program’s leadership team. The Manager will work closely with the physician Division Head for CCC to ensure delivery of high-quality services that are effective and sustainable. The Patient Care Manager, Complex Continuing Care is responsible for ensuring the delivery of excellent quality and safe care for patients/families, creating an exceptional patient experience, and for promoting a healthy work environment for staff, physicians and volunteers.

The successful candidate is a dynamic leader who has demonstrated a history of achieving results; is able to successfully lead a diverse, high performing team to achieve excellence; and is able to think critically and strategically in a complex and fast paced environment. Effective day to day management of the program is essential and includes but is not limited to: program operations, human resource management, ongoing quality improvement, and regular effective contact and interactions with external partners and stakeholders. The successful candidate will also consistently demonstrate Trillium Health Partners values of compassion, excellence and courage in leading the teams towards the growth of each service and the delivery of an exceptional patient and family experience.

Key Deliverables:

  • Plan, coordinate and effectively manage resources (human, financial, physical)
  • Lead program development, planning, implementation, evaluation and ongoing improvement
  • Collaborate with external and internal stakeholders in the development and delivery of an integrated and high-quality patient care and patient experience
  • Lead the development and sustainability of high performing teams
  • Data analysis of quality, utilization and outcome indicators
  • Maintain linkages and coordinate services with other community providers and regional partners
  • Actively participate in committees, both internal and external, addressing collaboration and coordination of services
  • Develop, plan and implement entrepreneurial and innovative ideas that enhance service delivery and cost effectiveness
  • Support innovation in processes, technology and people and action innovations that improve the quality and/or sustainability of services

THE IDEAL CANDIDATE

We are looking for a confident leader that can see the connection points and the big picture with respect to leading in a complex clinical environment, someone that sees solutions, not just the problems. The next Manager of the Moir will be a coach and mentor, an advocate for their team but will set high expectations and work to elevate practice and clinical standards. They will be empathetic and work with families to ensure they are engaged and included in their loved one’s care and will ensure positive and collaborative relationships between staff and visitors of the unit.

The successful candidate will possess the following experience, skills, attributes, and capabilities:

  • Completion of a baccalaureate degree in a health-related discipline; Master’s degree strongly preferred
  • Exceptional leadership and professional practice skills
  • 3 – 5 years related clinical experience
  • 3 – 5 years previous leadership/management experience; preference given to candidates with prior clinical and leadership experience with CCC or other complex long stay populations
  • Effective team building and creative thinking skills along with the ability to function effectively during periods of complexity, change and transition
  • Analytical, problem solving, and effective communication skills are essential
  • Advanced working knowledge of computers and relevant software
  • Demonstrated experience in program planning, implementation and development
  • A personal commitment to a team-based approach to planning and decision-making, combined with work and life balance with a willingness to seize personal accountability
