Position Summary:
Reporting to the Patient Care Manager, Toronto Grace Health Centre is currently seeking a Nurse Practitioner at our Scarborough location. The Nurse Practitioner is an active member of the interdisciplinary team that also includes a social worker, occupational therapist, and physiotherapist. The NP has clinical and leadership expertise in the provision of quality care for diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting. They will act as a leader and mentor for the professional nurses, allied health professionals, administrative and other supportive staff on the unit.
Responsibilities:
Provide client and family-centered care independently and in collaboration with other team members.
Demonstrate all the Advanced Nursing Practice Competencies outlined by the Canadian Nurses Association including clinical practice, research, leadership, consultation, and collaboration.
Conduct the patient care health assessment and diagnosis, therapeutic management, and health promotion and prevention of illness and injury.
Work in collaboration with the interdisciplinary team in a shared-care model of practice providing cross coverage.
Liaise regularly with external health organizations.
Support best practice in seniors’ care and other health care providers where appropriate.
Consult on patient/client care, providing advice, expertise, and consultation services.
Qualifications:
Currently registered with the CNO as a Registered Nurse in the Extended Class.
Currently have RNAO or NPAO membership with LAP or equivalent.
Additional post graduate education or certification in Mental Health and Addictions services / management related areas is a strong asset.
Minimum Three (3) years of relevant clinical experience, including experience working with clients with complex mental health illnesses.
Experience working with persons with Dementia or cognitive impairment.
Successfully completed a Primary Health Care Nurse Practitioner Program in Ontario or equivalent and hold a master’s degree in nursing with a Primary Health Care Nurse Practitioner Program (family all ages).
Your clinical focus will be with chronic disease management, geriatric nursing care, mental health, and addictions services where there is an emphasis on collaborative practice within an inter-professional milieu.
Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, push and pulling.
You have demonstrated clinical leadership skills and a commitment to continued participation in research.

1.0 FTE Full-time Permanent Nurse Practitioner supporting our Bloom Program
Bloom Clinic is a program of WellFort Community Health Services and is committed to providing treatment, care, and support to individuals living with, affected by, or at risk of Hepatitis C (HCV) and/or HIV across the Region of Peel. Bloom Clinic is committed to providing community-based testing, treatment, and support to people who are experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, and Indigenous community members.
The NP will integrate his/her knowledge of advanced nursing practice and theory, health assessment and diagnosis, therapeutic management, health promotion, disease prevention, and chronic disease management to provide primary health care to individuals, families, groups, and communities. The NP will work both independently and collaboratively with his/her clients, interdisciplinary team, and other healthcare providers in delivering high-quality clinical care and outreach activities.
While the proportion of time allocated to different functions may vary according to service needs, the advanced nursing role encompasses a combination of direct care, health promotion, outreach, education, advocacy, community development, program development, and evaluation.
WellFort is a non-profit, values-based organization that provides health promotion and prevention programs, primary and oral health care and works with local residents and other agencies to increase community capacity.
What you will do:
Perform a focused and/or holistic health assessment, health history, and physical examination based on the client needs and across the lifespan (e.g., prenatal, postpartum, well-baby, infants, adolescents, adults, and older adults)
Using strong clinical reasoning skills, synthesize health assessment information and evidence-informed practice to formulate differential diagnoses and communicate normal/abnormal findings to the clients
Consultation with other health professionals as required in establishing a diagnosis and plan of care.
Order and interpret screening, diagnostic procedures, and other investigations within the CNO scope of practice and ensure timely follow-up
Other areas of focus will include Therapeutic Management, Health Promotion and Education, Advocacy and Community Development, Administration, evidenced based practice including documentation and program reporting, Professional Development and Contribution to team and centre activities
Provide care delivery at any of the office sites as program needs dictate.

What you will need:
Undergraduate degree in Nursing required; Masters of Science in Nursing (MScN) preferred

Bloom Clinic is committed to providing community-based testing, treatment, and support to people who are experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, and Indigenous community members.
The NP will integrate his/her knowledge of advanced nursing practice and theory, health assessment and diagnosis, therapeutic management, health promotion, disease prevention, and chronic disease management to provide primary health care to individuals, families, groups, and communities. The NP will work both independently and collaboratively with his/her clients, interdisciplinary team, and other healthcare providers in delivering high-quality clinical care and outreach activities.
While the proportion of time allocated to different functions may vary according to service needs, the advanced nursing role encompasses a combination of direct care, health promotion, outreach, education, advocacy, community development, program development, and evaluation.

What you will do:
Perform a focused and/or holistic health assessment, health history, and physical examination based on the client needs and across the lifespan (e.g., prenatal, postpartum, well-baby, infants, adolescents, adults, and older adults)
Using strong clinical reasoning skills, synthesize health assessment information and evidence-informed practice to formulate differential diagnoses and communicate normal/abnormal findings to the clients
Consultation with other health professionals as required in establishing a diagnosis and plan of care.
Order and interpret screening, diagnostic procedures, and other investigations within the CNO scope of practice and ensure timely follow-up
Other areas of focus will include Therapeutic Management, Health Promotion and Education, Advocacy and Community Development, Administration, evidenced based practice including documentation and program reporting, Professional Development and Contribution to team and centre activities
Provide care delivery at any of the office sites as program needs dictate.

What you will need:
Undergraduate degree in Nursing required; Masters of Science in Nursing (MScN) preferred
Council of Ontario University Program in Nursing (COUPN) Primary Health Care Nurse Practitioner Certificate required.
Current registration with the CNO as a Registered Nurse Practitioner with NP-HPCS specialty certificate required
Current membership with RNAO or NPAO and evidence of liability insurance required
A minimum five years of nursing experience in a variety of primary care settings one of which must be community health.

What you’ll do
Perform role and specialty specific clinical workflow assessments and translates requirements to digital health/information technology leads. Liaise with multiple clinical groups, project teams, and members of the organization and peer health authorities to assess technology gaps and clinical needs for providers and clinical users of the electronic health record system. Provide subject matter expertise in system design and evaluation of clinical content, troubleshooting and end–user support.
Liaise with the Digital Health department, organizational programs/departments and partner affiliates as the role and informatics specialty specific expert and resource on system wide practice and workflow change initiatives, practice innovation and redesign, documentation/nomenclature standards and the electronic health record.
Champion a collaborative team-based framework that incorporates resources from other departments, programs, and vendor partners as it relates to clinical systems implementations. Lead and participates in steering committees and taskforces for PHSA clinical informatics and information technology initiatives.
Lead in the development of organizational and/or departmental clinical change management policies and procedures as they relate to the electronic health record and clinical practice process redesign as it pertains to the automation of professional practice. Lead the identification and assists in the development of new and changing role and specialty specific functions and processes in the department/organization as they relate to the use of clinical information systems.
Lead the system design life cycle in the areas of design, content development, validation, implementation, stabilization and evaluation of electronic health information systems and their associated data dictionaries, databases, tables, reports and related standards, protocols and procedures to support clinical uptake of electronic health and information records. Participate in custom report development and data analysis for decision-making.
Provides leadership and/or coaching to nurses and other clinical staff based on knowledge and informatics experience on incorporating clinical informatics advances and electronic health records developments into practice.
Engages in the planning, coordination, scheduling, implementation and evaluation of upcoming releases including impact analysis, development of release notes and liaising with other subject matter experts to support development and delivery of education for end users to enhance adoption, including use of clinical decision support tools within the clinical workflows to support timely decision making.
Plans and leads adoption training sessions such as, workflow review sessions and one-on-one support sessions related to appropriate, safe and efficient use of clinical information systems/applications including applicable policies and related legislation regarding access to electronic health records.
Identifies innovative approaches and standards for information capture, storage and retrieval that ensure the management of and secure access to personal electronic health information in collaboration with security management and application analysts. Participates in data remediation/data integrity activities and audit processes to ensure quality data collection and system efficiency.
Documents and communicates clinical informatics standards to information technology leadership, staff, providers, project directors, project managers and the appropriate user community representatives such as staff from other portfolios within PHSA, colleagues from peer health authorities and vendors. Maintains familiarity with health care industry developments in clinical informatics, participates in advising on implementation of related standards, tools, and processes within PHSA.
What you bring
Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM), formally known as British Columbia College of Nursing Professionals (BCCNP).
Three (3) years’ experience in a Clinical Informatics environment, including experience facilitating or participating in Clinical and Systems Transformation (CST) implementation, or an equivalent combination of education, training and experience.
May require the use of a vehicle for which mileage is reimbursed in accordance with the current Collective Agreement.

Lead in the development of organizational and/or departmental clinical change management policies and procedures as they relate to the electronic health record and clinical practice process redesign as it pertains to the automation of professional practice. Lead the identification and assists in the development of new and changing role and specialty specific functions and processes in the department/organization as they relate to the use of clinical information systems.
Lead the system design life cycle in the areas of design, content development, validation, implementation, stabilization and evaluation of electronic health information systems and their associated data dictionaries, databases, tables, reports and related standards, protocols and procedures to support clinical uptake of electronic health and information records. Participate in custom report development and data analysis for decision-making.
Provides leadership and/or coaching to nurses and other clinical staff based on knowledge and informatics experience on incorporating clinical informatics advances and electronic health records developments into practice.
Engages in the planning, coordination, scheduling, implementation and evaluation of upcoming releases including impact analysis, development of release notes and liaising with other subject matter experts to support development and delivery of education for end users to enhance adoption, including use of clinical decision support tools within the clinical workflows to support timely decision making.
Plans and leads adoption training sessions such as, workflow review sessions and one-on-one support sessions related to appropriate, safe and efficient use of clinical information systems/applications including applicable policies and related legislation regarding access to electronic health records.
Identifies innovative approaches and standards for information capture, storage and retrieval that ensure the management of and secure access to personal electronic health information in collaboration with security management and application analysts. Participates in data remediation/data integrity activities and audit processes to ensure quality data collection and system efficiency.
Documents and communicates clinical informatics standards to information technology leadership, staff, providers, project directors, project managers and the appropriate user community representatives such as staff from other portfolios within PHSA, colleagues from peer health authorities and vendors. Maintains familiarity with health care industry developments in clinical informatics, participates in advising on implementation of related standards, tools, and processes within PHSA.
What you bring
Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM), formally known as British Columbia College of Nursing Professionals (BCCNP).
Three (3) years’ experience in a Clinical Informatics environment, including experience facilitating or participating in Clinical and Systems Transformation (CST) implementation, or an equivalent combination of education, training and experience.
