An exciting opportunity exists for an Integrated Care Manager to lead two interdisciplinary teams: St. Paul Home Care and Long Term Care. We also offer a swimming pool, two arenas, a public library, fitness centres, and a UFO landing pad to keep community members very active, supporting St. Paul’s motto that we’re “a people kind of place.” Reporting to the Area Manager, Seniors Health, the Integrated Care Manager is responsible and accountable for direction, team leadership and organization over the day to day activities of the St. Paul Long Term Care and the St. Paul Home Care departments. You will be responsible for and accountable for making unit-level decisions and having direct leadership responsibility for contributing to the achievement of the goals of the client and resident care service. This position will require strong relationship building abilities that include facilitation, collaboration and consultation skills. If you can utilize persuasion and influence in order to successfully lead the Long Term Care and Home Care departments, this position may be for you.

Description:

As the Integrated Care Manager - Home Care and Long Term Care, you will be responsible for providing leadership to an interdisciplinary team of healthcare professionals dedicated to providing leading care to Albertans. Your leadership, direction and organization of St. Paul Home Care and St. Paul Long Term Care will be in support of the vision, mission and business plan of AHS. As the Integrated Care Manager, you will be making key decisions and establishing and contributing to the achievements and goals of the Seniors Health portfolio. As the Manager, you will be responsible for developing and maintaining strong working relationships with key stakeholders, facilitating change management, promoting team development, teamwork and coordinating staffing and clinical work as identified by the Area Manager. You will be responsible for maintaining an effective framework for the operations of patient care services in order to facilitate the delivery of efficient management, quality assurance and utilization of information in this field. As the successful candidate, you will have experience successfully communicating with different stakeholders and demonstrate a client-centered approach. We are also looking for someone who has the ability to develop creative approaches to client and resident care and who demonstrates sound critical thinking and ethical decision making.

Required Qualifications:

A minimum of a bachelor’s degree in healthcare profession, healthcare management or related field. A minimum of 2 to 5 years in a management role in a healthcare organization of significant size, complexity and diversity.

Demonstrated ability to manage change and create innovative solutions for complex and diverse issues. Ability to manage human, financial and physical resources within an operating environment. Proven ability to foster partnerships and to achieve organizational goals within an organization and ideally managing within a unionized sector. Strong track record in establishing and maintaining effective working relationships with internal and external stakeholders. Effective leadership abilities combined with a strong commitment to operating within a team environment and the ability to motivate and inspire other to achieve common goals. Excellent analytical and decision-making skills. Ability to plan, organize and manage short and long term plans and projects and deliver customer-focused results. Ability to foster a climate of cooperation amongst, and build solid relationships with external stakeholders.

Salary Range: $36.68 - 62.91
