What we need:

Provide a nursing assessment (including assessments of physical, psychological, social, cultural and spiritual needs). Provide a full scope of nursing knowledge and skills in geriatric medicine, acute medicine and complex medicine. Demonstrate strong evidenced based and family centered approach to holistic care for all patients. Strong clinical skills and expertise to independently follow physician orders to implement care and treatments and provide nursing care to acutely ill medical patients and complex and frail elderly patients. Work with the interprofessional team to provide a holistic plan of care, communicate with the patient and family/caregiver, explore patient needs and plan with them to transition back to the community seamlessly. Use nursing judgment and expertise to organize sequencing of community supports, physician assessments, diagnostic tests and procedures where required. Keeping abreast of the patient’s response to community care providers, interventions and modifying the care plan accordingly in communication with the physician and the Hospital to Home care team. Provide a comprehensive report when transferring accountability of a patient’s care to another health care professional, community agency and/or facility. Establish a caring and therapeutic relationship with patients; demonstrate trust, respect, honesty and provide counseling as required. Expedite access to services and resources to improve continuity and coordination of care between hospital and home. Empower patients and their caregivers through the development of trusting relationships, and through providing information and education to facilitate decision making in order to enhance the patient’s choice of the best alternatives for supportive care in the community



The successful candidate must be available to work from 0900 hrs to 2300 hrs Monday through Friday and 1000 hrs to 1800 hrs Saturday and Sunday


Required Skills:

  • Current Certificate of Registration with the College of Nurses with BScN required
  • Masters prepared, or willing to work towards
  • Current BCLS certification
  • Minimum of 5 years of experience in medicine, geriatrics and community nursing
  • Previous experience in an outpatient setting is an asset
  • RNAO membership is preferred
  • Excellent physical assessment and patient education skills
  • Working knowledge of computer technology
  • Demonstrated commitment to a patient/family centred care environment
  • Demonstrated ability to multi task in a fast paced environment
  • Demonstrated excellent critical thinking and problem solving skills
  • Demonstrated ability to work effectively within an inter professional team as well as independently
  • Well developed organization, communication, interpersonal and teaching skills
  • Evidence of ongoing professional development, continuing education and leadership experience
  • Demonstrated good attendance record with the ability to maintain this same standard
  • Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required
